학령기 자녀를 둔 성인 ADHD 성향 어머니를 위한 인지행동치료 프로그램의 효과: 자녀 양육 관련 사고 및 행동의 변화를 중심으로

초록

본 연구의 목적은 학령기 자녀를 둔 성인 ADHD 성향 어머니의 자녀 양육 경험에 대해 살펴보고, 인지행동치료 프로그램이 자녀 양육에 미치는 영향에 대해 인지적 특성과 양육행동 특성을 토대로 질적 변화를 살펴보는 것이다. 연구 대상은 성인 ADHD 성향을 보이는 학령기 자녀를 둔 어머니로 본 프로그램에 자발적으로 참여를 희망한 6명이며, 개인치료 회기 12회(주 1회, 회기당 90분씩)와 모-자녀 공동 회기 2회(개인치료 7회기와 8회기 후 각각 40분씩), 총 14회기 프로그램이 진행되었다. 자료 수집은 어머니를 대상으로 프로그램 진행 전반에 걸쳐 이루어졌고, 양육의 대상인 학령기 자녀가 지각한 변화를 살펴보기 위해 개별 면담을 진행하였다. 수집된 자료는 프로그램 주요 내용을 중심으로 질적 사례연구 방법의 절차를 통해 분석하였다. 분석 결과, 학령기 자녀를 둔 성인 ADHD 성향을 보이는 어머니는 성인 ADHD 증상으로 인해 자녀 양육 장면에서 비합리적인 자녀 양육 관련 사고와 부적응적 양육행동을 나타내게 되며, 성인 ADHD 성향 어머니를 위해 특화된 인지행동치료 프로그램을 통해 자녀 양육 관련 왜곡된 인지를 적응적으로 재구조화하고 실행기능기반으로 양육기술을 습득함으로써 학령기 자녀 양육에서 긍정적으로 변화됨을 확인하였다. 또한, 학령기 자녀도 어머니와 동일한 양육 항목들에서 긍정적인 변화를 느끼고 있음을 확인하였다.

Abstract

This study aims to explore the parenting experiences of mothers with adult ADHD tendencies who are raising school-aged children and to assess the effect of a cognitive behavior therapy (CBT) program tailored for these mothers on their parenting. The research delves into qualitative changes in cognitive and behavioral characteristics related to parenting, as well as the perceived parenting behaviors from the perspective of the school-aged children of mothers with adult ADHD tendencies. The study involved six mothers exhibiting adult ADHD tendencies, each with school-aged children, who voluntarily opted to participate in the program. The program spanned 14 sessions in total, including 12 individual therapy sessions (held weekly for 90 minutes per session) and two joint mother-child sessions (40 minutes each, held after the 7th and 8th individual therapy sessions). Data collection for the mothers occurred throughout the duration of the program, while data from the children were gathered through individual interviews. The collected data were analyzed using qualitative case study methods, focusing on the content extracted in alignment with the program’s primary objectives. The analysis showed that mothers with adult ADHD tendencies who are raising school-aged children display irrational parenting-related thoughts and negative parenting behaviors due to their adult ADHD symptoms. Through the CBT program tailored for these mothers, it was observed that they were able to adaptively restructure their distorted parenting- related cognitions and acquire parenting skills grounded in executive function principles, resulting in positive changes in their parenting of school-aged children. Furthermore, it was found that their school-aged children also recognized positive changes in the same parenting areas as their mothers.
