Abstract
The purpose of this study is to establish the inter-rater reliability as well as the concurrent validity of the Tinetti-Balance scale that translated into Korean in chronic stroke patients. The subjects participated in this study were Thirty- eight patients with hemiparesis being able to walk independently. The Tinetti- Balance scale was administered in 2 testing sessions 2∼3 days apart. Inter- rater reliability(intraclass correlation coefficient ICC2,1) and Kappa coefficient was used to examine the level of agreement between raters. Absolute reliability indices, including the Bland and Altman method, the standard error of measurement(SEM), and the Minimal Detectable Change(MDC). Concurrent validity was examined by correlating the total score of Tinetti-balance to the gait speed(10m walking test), dynamic standing balance(affected/ non-affected One Leg Standing Test, OLST), strength of lower extremity(Sit to Stand test, STS), function of motor(Fugl Meyer- Upper/Lower extrmity, FM-UE/LE). The overall inter-rater reliability of the total score was high ICC(2,1)=.914(95% CI=.834~.955)(very reliable), the Kappa coefficient =.62~.83(substantial∼good). The ICC of the individual items ranged from .768 to .916(95 % CI=.553~.956)(reliable∼very reliable). The SEM and MDC of the Tinetti-balance representing were 1.02 and 2.83, respectively, exhibiting the smallest change threshold that indicates a real improvement for a group of individuals and a single individual. The total score of Tinetti-balance correlated with 10m gait speed(r=-.74), affected/non-affected OLST(r=-.72∼.76), STS(r=-.73), FM- UE/LE(r=64∼.76) (p<.01). The Tinetti-Balance is a reliable and valid for assessing the balance status for individuals with stroke. The thresholds of Tinetti-Balance scale to detect real change are acceptable in research and clinical settings.
