목적: 본 연구의 목적은 복식 전자궁적출술 전후 비뇨기계, 하부 위장관계, 그리고 성적 기능의 변화를 비교하고 여성의 자각력과 만족도를 조사하기 위함이다. 연구 방법: 본 연구는 비암종성 병변의 치료를 위해 복식 전자궁적출술을 시행한 31세에서 65세 사이의 여성 89명을 대상으로 한 전향적 조사였다. 89명의 여성은 수술 전과 수술 후 1개월, 3개월, 6개월에 면접조사에 임하였다. 수술 후에 환자가 진술하는 비뇨기 증상, 하부 위장관 증상, 그리고 성적 기능과 환자의 만족도에 대해 조사하였고, P값 0.001 이하만 통계적 의미가 있다고 하였다. 결과: 자궁적출술을 시행 받게 되는 가장 흔한 원인은 비정상 자궁 출혈이었다. 자궁적출술 후 이차적인 합병증은 많지 않았다. 자궁적출술은 골반통과 복통, 위장관계 증상, 비뇨기계 증상을 호전시킨다. 자궁적출술 후 극치감의 빈도는 감소했으나 다른 성적 요소들의 기능 감소는 그리 크지 않았다. 자궁적출술 후 만족도는 매우 높았다. 결론: 양성질환 치료를 위한 복식전자궁적출술은 비뇨기 증상, 하부 위장관계 증상을 호전시키고, 심한 성기능의 감소는 초래하지 않아 환자에게 높은 만족도를 준다.
Abstract
Objective: Our purpose was to investigate women’s perception and satisfaction before and after hysterectomy comparing urinary, lower gastrointestinal, and sexual function. Methods: It was a prospective study of 89 women ages 31-65 years undergoing total abdominal hysterectomy for nonmalignant conditions. These women were interviewed before surgery and 1, 3 and 6 months later. Patients-reported symptoms of urinary, lower gastrointestinal, and sexual function and woman’s satisfaction of hysterectomy were assessed. Only P≤0.001 was considered statistically significant. Results: The most common reason of patients for hysterectomy was abnormal uterine bleeding. Secondary complication after hysterectomy was negligible. Hysterectomy has got lead to the improvements in pelvic/abdominal pain, gastrointestinal symptoms, and urinary symptoms. The frequency of orgasm was reduced, but other sexual variables were not changed significantly. The level of satisfaction after hysterectomy was very high. Conclusion: Abdominal hysterectomy for benign conditions improves urinary and lower gastrointestinal function with no consistent changes in sexual function. Hysterectomy gives patients high degree of satisfaction as well as marked improvement of quality of life.
