Abstract

Background: The demand for blood products continues to rise, while supply declines, highlighting the need for appropriate blood utilization. Transfusion risks, including allogeneic immune reactions and infections, further reinforce this necessity. This study aimed to evaluate transfusion indications and assess the appropriateness of blood product use at a single center. Methods: A retrospective review of blood product orders from January to December 2023 was performed. Patient transfusion records and co- morbidity data were analyzed using an electronic audit program based on national transfusion guidelines. Results: The adequacy rate for packed red blood cell transfusions was 67.98%, with only 51.07% adequacy among patients with hemoglobin 7-10 g/dL. Platelet transfusions showed the highest adequacy (97.58%) but declined to 66.13% when platelet counts were 50,000-100,000/μL. Fresh frozen plasma transfusions demonstrated 86.58% adequacy, although 69% of cases did not satisfy all prothrombin time, activated partial thrombin time, and fibrinogen criteria. Significant differences in transfusion adequacy were observed among internal medicine, surgery, and emer- gency/ICU departments (P = 0.000). Return and disposal rates were 1.83% and 0.65%, respectively, primarily due to patient improvement. Conclusions: Improving transfusion appropriateness requires institution-specific guidelines and systematic evaluation of inappropriate transfu- sion causes. National-level support systems are essential to optimize blood product utilization.
