Abstract
Background: The Sebia Capillarys 3 OCTA platform, recently introduced for clinical laboratory use, is an analytical system designed for protein electrophoresis and immunotyping. These assays are critical for the diagnosis and monitoring of conditions such as multiple myeloma, monoclonal gammopathy of undetermined significance, and amyloidosis. In this study, we evaluated the analytical performance of the Capillarys 3 OCTA system. Methods: Precision and linearity were assessed using control materials and patient samples in accordance with CLSI guidelines. Comparative analysis was performed between the Capillarys 3 OCTA and Capillarys 2 FLEX-PIERCING systems using patient samples, including those with monoclonal proteins. Reference interval verification and carry-over evaluation were also conducted. Results: Within-laboratory imprecision for the six protein fractions was ≤ 2.0% for albumin and ≤ 10.0% for the remaining fractions at both low and high concentrations. Linearity of IgG-kappa and IgM-lambda monoclonal proteins was confirmed. Comparative analysis with the Capillarys 2 FLEX-PIERCING system showed that mean differences and 95% confidence intervals for serum protein fractions were within the 10.0% critical difference threshold. However, in urine protein electrophoresis, the albumin and alpha-2 fractions exceeded this threshold. All isotypes in serum and urine immunotyping were concordant between the two instruments. Manufacturer-provided reference intervals were verified, and no carry- over was observed. Conclusions: The Capillarys 3 OCTA met predefined imprecision criteria. While quantification of monoclonal proteins showed acceptable agree- ment between the two systems, variability in specific protein fractions indicates that caution is warranted when interpreting electrophoresis results.
