Abstract
This study aims to examine the interactive effects of attention-deficit/hyperactivity disorder (ADHD) symptoms and perceptions of mental health service use on suicide risk among elementary school students. Data were drawn from the 2021 National Survey on Mental Health of Adolescents conducted by the National Youth Policy Institute of Korea, focusing on a subsample of 2,039 elementary school students. The findings are as follows. First, compared to children in the low-risk ADHD group, those in the borderline and high-risk ADHD groups had higher proportions of boys, greater exposure to school violence, and lower levels of school adjustment. Second, suicide risk was 2.58 times higher in the borderline group and 4.61 times higher in the high-risk group than in the low-risk group. However, perceptions of mental health service use alone were not directly associated with suicide risk. Third, when examining the combined effects of ADHD risk and perceived burden of service use, all groups except for the reference group (low-risk ADHD × no perceived burden) showed significantly elevated suicide risk, with relative risks ranging from 2.22 to 6.05. Based on these findings, the study offers policy and practice recommendations to promote mental health service utilization among children at heightened risk for ADHD and suicidality, with the ultimate goal of enhancing mental health outcomes and preventing suicide.
