공공 및 민간 의료기관에 대한 공급자 유인 수요 및 의료비 분석

초록

본 연구에서는 공공 및 민간 의료기관을 대상으로 공급자 유인수요가 존재하는지, 그리고 신포괄수가제의 시행이 의료기관의 진료비와 의료 이용에 영향을 미치는지에 대해서 분석하였다. 이를 위해 국민건강보험공단에서 구득한 공공 및 민간 의료기관의 2009년부터 2017년까지 자료를 이용하여 패널분석을 시행하였는데, 분석대상인 전체 의료기관, 공공 및 민간 의료기관 및 공공의료기관 만을 대상으로 각각 시행하였다. 분석 결과, 전체 의료기관을 대상으로 한 분석에서 공급자 유인수요가 발생하고 있는 것을 발견하였으며, 공공 및 민간 의료기관에 대한 분석에서는 민간의료기관에서만 공급자 유인 수요가 발생하는 것을 확인하였다. 그러나 공공의료기관을 대상으로 신포괄수가제 시행 확대 효과를 파악하기 위해 시행한 패널분석에서는 유의한 결과를 도출하지 못하였다. 본 연구는 공공 및 민간 의료기관과 같은 설립형태의 차이에 따라 공급자 유인수요의 발생이 달라질 수 있다는 것을 파악하였다는 데서 의의가 있다고 하겠다.

Abstract

In this study, we analyzed the existence of the supplier-induced demand in the public and the private hospitals and whether New Diagnosis Related Group Based Payment affected the supplier-induced demand. This study used the public and the private hospitals’ data between 2009 and 2017 from National Health Insurance Service to implement the panel study on the entire hospitals, the public and the private hospitals respectively, and the public hospitals alone. The analysis on the entire hospitals showed that the supplier-induced demand occurred in the hospitals. Estimation on the public and the private hospitals respectively found that the supplier-induced demand occurred in the private hospitals only. This study is meaningful in the sense that we analyzed the public and the private hospitals respectively to find that there occurred the supplier-induced demand in those hospitals.
