비중증 과잉 비급여 개선방안 연구: 의원급 비급여 진료비용 공개제도의 효과 분석을 중심으로

국내 비급여 진료비의 지속적 증가는 국민 의료비 부담 상승을 포함한 다양한 사회적 문제들을 초래할 수 있다. 특히, 비중증 비급여 항목은 실손의료보험과 결합하여 과잉 진료되고 있어, 이에 대한 집중관리가 시급한 실정이다. 본 연구는 정부에서 과잉 비급여 진료를 완화하기 위해 시행한 비급여 진료비용 공개제도의 실효성을 분석하고자 하였다. 실증분석을 위해 패널 사건사 분석(Panel Event Study Analysis)을 활용하여 주요 결과를 도출하였다. 연구 결과 첫째, 진료비용 공개에도 불구하고 의원급 비급여 진료비는 정책 시행 3년 이후 상승하였다. 둘째, 비중증 항목의 경우, 정책 시행 직후 가격의 편차가 중증 항목보다 심화된 것을 알 수 있었다. 셋째, 의료 기관의 의료인 수와 환자 수는 비급여 진료 가격 형성에 영향을 미치고 있음을 확인할 수 있었다. 본 연구를 통해 비급여 진료비용 공개제도가 진료비 상승 완화에 미치는 영향이 제한적이라는 것을 확인할 수 있었다. 따라서 비중증 과잉 비급여를 효과적으로 관리하기 위해 의료소비자의 알 권리를 위한 ‘비급여 사전 설명제도’ 도입, 혼합진료 금지, 비급여 항목의 표준화와 같은 다각적인 방법들이 동원되어야 한다.
Abstract
The continuous increase in uncovered medical expenses in Korea can lead to various social problems, including an increase in the burden of national medical expenses. In particular, uncovered medical items are treated in excess in combination with indemnity medical insurance, so intensive management is urgent. This study attempted to analyze the effectiveness of the uncovered medical expense transparency policy implemented by the government to alleviate excessive uncovered medical treatment. For empirical analysis, main results were derived using Panel Event Study Analysis. As a result of the study, first, despite the disclosure of medical expenses, uncovered medical expenses at the clinic level have risen since 3 years after the policy was implemented. Second, in the case of uncovered medical items, it was found that the price deviation was more severe than that of the severe items immediately after the policy was implemented. Third, it was confirmed that the number of medical personnel and patients in medical institutions have an effect on the formation of uncovered medical treatment prices. Through this study, it was found that the impact of the uncovered medical expense disclosure system on the reduction of the increase in medical expenses was limited. Therefore, in order to effectively manage the uncovered medical services, various methods such as a preliminary explanation uncovered medical expenses for medical consumers’ right to know, the prohibition of mixed reimbursement treatment, and the standardization of uncovered medical items should be mobilized.
