민간의료보험 가입이 의료서비스 이용량에 미치는 영향: 패널 포아송 회귀모형 분석

본 논문은 한국복지패널의 2007년~2017년간의 자료를 이용한 패널 포아송 회귀모형의 추정을 통해, 민간의료보험의 최초 가입 및 추가 가입이 건강검진 횟수, 외래진료 횟수, 입원 횟수 및 일수 등의 의료서비스 이용량에 미치는 영향을 분석하였다. 추정 모형 내에 고정효과 및 시간효과의 포함과 시차 설명변수의 사용을 통해 민간의료보험 가입과 의료서비스 이용량 간의 내생성의 문제를 최대한 통제하고자 하였다. 추정결과, 민간의료보험의 최초 가입은 고려한 의료서비스들의 이용량을 통계적으로 유의하게 증가시키는 것으로 나타났으나, 추가 가입은 외래진료 횟수에만 긍정적 영향을 나타냈다. 또한, 입원 서비스 이용에 대한 영향에 있어서 민간의료보험 가입은 입원 횟수의 증가를 통해서 입원 일수를 증가시키고 있음을 확인하였다. 이러한 결과들은 민간의료보험의 가입이 의료서비스 이용량의 증가를 통해 의료서비스 제공에 요구되는 한정된 인력과 자원의 점유를 증가시키는 결과를 낳고 있음을 보여준다. 반면에 추가로 가입하는 민간의료보험은 그러한 영향이 최초 가입과 비교하면 제한적인 것으로 볼 수 있다. 이러한 의료서비스 이용량의 비효율적인 증가는 의료서비스 시장에서의 자원 배분 왜곡을 초래하고, 국민건강보험의 재정부담을 증가시키는 역효과를 발생시킬 수 있는 만큼 다방면의 세심한 정책적 관심이 요구된다.
Abstract
Based on the estimation of the panel Poisson regression model using the Korean Welfare Panel for the period of 2007-2017, this paper analyzed the effect of initial and additional subscriptions of private medical insurance on the number of health checkups, outpatient care, and hospitalizations as well as the days of hospitalization. By including the fixed and the time effects in the panel estimation model as well as by using the lagged explanatory variable, we tried to control the endogeneity problem between the private health insurance subscription and the medical service usage. Estimation results show that initial subscription to private medical insurance significantly increases the usage of all the medical services considered but the additional subscription have a significant positive effect only on the number of outpatient care. In addition, the results also show that the initial subscription of private medical insurance increases the days of hospitalization through the increase in the number of hospitalizations. These results imply that private medical insurance subscriptions result in the increase in medical service usage, which, in turn, results in the increased share of very limited personnel and resources required to provide medical services. On the other hand, additional subscription of private medical insurance coverage seems to have a limited such effect compared to initial subscription. Such an inefficient increase in the use of medical service requires careful policy attention as it results in distortions of resource allocation in the medical care market, and in the adverse effects of increasing the financial burden of national health insurance.
