Abstract

The principle of patient autonomy, which is increasingly being emphasized in the medical field today, has not sufficiently discussed and its scope of consideration has been limited to an area of individual decision or choice itself. In medical decision making, the patient is a prescriptive subject who has the ultimate authority to make decisions about his or her health and body, but is also vulnerable and dependent because of medical treatment and care by others. However, the discourse on autonomy used in the medical field does not consider the nature of the patient but reduces discourses of autonomy to the choice or decision of individuals with reasonable judgment ability. I am looking for the cause of this tendency in the conventional way of discussing the concept and basis of autonomy, which I call the autonomy competence model. According to the autonomy competence model, the concept of autonomy is an individual ability to think independently of himself or herself from the outside, which is an individual competency. If such a perspective is put into the medical decision making process, physical vulnerability and interdependence of the patients in the medical situation are hard to be considered properly. In addition, the dualistic approach of the autonomic competence model has limitations in functioning as a framework to reflect the characteristics of diverse and complex medical decision-making processes. Furthermore, this fixed frame of competent model of autonomy would result in unintended negative consequences. Under this background, this paper critically examines the standard autonomy discourse from the viewpoint of the autonomy competence model and finds alternatives to overcome these problems, which are caused by the autonomy competence model being applied to the medical decision-making process. As one possibility, I focus on the theory of relational autonomy that feminism attempts. Various attempts at the theory of relational autonomy can present new perspectives in expanding the concept of autonomy in the medical field. In particular, it should be able to better explain the nature of the patient, who is a party to medical decision making, and to closely observe the characteristics of the complex medical decision-making process. On the other hand, the various approaches of the relational autonomy theory without a single point of view reveal limitations in systematically constructing the entire discourse of patient autonomy. Therefore, further research will be required to develop an alternative autonomy model that can be systematically applied to the medical field while accepting the advantages of relational autonomy.
