본 논문은 후기고령자 의료제도 도입과정을 사례로 일본에서 작동하고 있는 복지정치의 메커니즘을 분석해 보는 데 목적이 있다. 후기고령자 의료제도는 75세이상의 고령자를 대상으로 하는 독립의료보험 제도로서, 고령자의 부담을 새롭게발생시키는 것이었음에도 불구하고, 2006년 별다른 저항 없이 도입되었다. 그 배경에는 첫째, 제도적 측면에서의 수상의 권한확대와 고이즈미에 대한 높은 지지율이 존재한다. 둘째, 의료비 부담증가를 구조개혁의 이슈로 재정의 하고, 개혁의부담을 일부 계층에 집중하게 하는 비난회피의 전략이 대중적 저항을 사전에 차단하였다. 셋째, 복지정치가 미디어를 중심으로 전개되는 가운데, 제도 도입 당시미디어의 후기고령자 의료제도에 대한 관심은 낮았고, 이에 따라 유권자의 정책적인지도 역시 낮았다. 행정개혁의 효과로 복지정치의 주도권이 관료에서 정치가로옮겨지게 되었지만, 사회집단에 의한 일상적 복지정치가 활성화되지 않는 한 일본의 ‘복지정책의 탈정치화’는 지속될 것으로 보인다.
Abstract
This study aims to explore the mechanisms of welfare politics in Japan viathe introduction process of medical care system for the elderly aged 75 and over. Medical care system for the elderly aged 75 and over is an independent medicalinsurance system which is designed to burden the elderly population. However, itwas introduced in 2006 with modest resistance. Such introduction of aburdensome welfare policy can be explained with three backgrounds. First is theexpansion of the prime minister’s authority and the high approval rating for theKoizumi Cabinet. Secondly, the strategies of blame avoidance, redefining theissue of increasing medical costs as an structural reformation and focusing on acertain social stratum for the burden of the reform, block out social resistance inadvance. Thirdly, while welfare politics is developed via media, the press had lowinterest toward the medical care system for the elderly aged 75 and over its initialphase. Therefore, the political awareness of the voters was low. De-politicizingof the welfare policy in Japan will be continued unless routine social groups areactivated.
