초록

본 연구는 ADHD아동의 신경심리적 인지 특성을 알아보기 위하여 ADHD아동 집단과 일반아동 집단의 인지요인과 실행기능을 비교하였다. 일반학교에 재학 중인 4-6학년 ADHD아동과 일반아동 각 30명을 대상으로 신경심리 검사를 실시하였으며, 연구에 사용된 검사도구는 ‘K-WISC-Ⅲ 지능검사’와 ‘아동용 Rey-Kim 기억 검사’,‘아동용 Kims 전두엽-관리기능 신경심리검사’이다. 연구결과 ADHD아동들은 인지요인에 있어서 일반아동들에 비해 전반적으로 낮은 점수를 얻었으며, 그 중 언어이해와 지각조직 요인은 일반아동과 유의미한 차이가 없었다. 그에 반해 주의집중과 기억, 처리속도 요인에서는 유의미한 차이가 있었는데 그 중 가장 큰 차이를 보인 것은 주의집중 요인으로 나타났다. 인지요인별 상관을 알아본 결과 일반아동 집단에서는 기억과 주의집중, 지각조직, 처리속도 요인 간에는 상관이 없었고, 나머지 요인들 간에는 통계적으로 유의미한 상관관계가 있었다. ADHD아동 집단에서는 주의집중과 지각조직, 지각조직과 기억, 처리속도, 처리속도와 지각조직 요인간에는 상관이 없었고, 나머지 요인들 간에는 통계적으로 유의한 상관관계가 있었다. 또한 실행기능을 비교해 본 결과 ADHD아동들은 일반아동에 비해 실행지능이 낮았으며, 실행기능들 중에서 ‘스트룹 검사’ 기능이 가장 낮았다. 질적 점수에서는 스트룹 검사의 ‘간섭억제지표’와 도안 유창성 검사의 ‘상식 일치도’및 ‘빠진 곳 찾기 일치도’에서 낮은 점수를 보였다. 이러한 결과는 ADHD아동이 일반아동에 비해 주의집중, 지각조직과 기억, 처리속도에서의 수행력이 낮아서 제시된 외부의 단서나 정보에 대한 주의력이 낮고, 작업기억과 조직 능력, 방해자극에 대한 반응억제와 정보처리능력이 낮음을 알 수 있다. 결국 ADHD아동들의 이러한 실행기능 특성을 고려한 다양하고 흥미있는 중재 방법과 교수 전략을 구안할 필요성이 제기된다.

Abstract

This study aims to find the neuropsychological characteristics of children with Attention Deficit Hyperactivity Disorder(ADHD) by comparing the cognitive factor and executive function between a group of children with ADHD and the other group of non ADHD(n-ADHD). Neuropsychological examination was carried out on 30 children each from the group of ADHD and n-ADHD in their fourth to sixth grade in public schools. The examination method used in the study is ‘K-WISC-Ⅲ Intelligence Test', ‘Rey-Kim Memory Test for children', and ‘Kim's Frontal-Executive Function Test for children'. The study result shows that there was a difference in the cognitive factor of ADHD compared with n-ADHD. Among the factors, the level of VCI was the highest, and that for FDI was the lowest. As a result of examining the relationship between factors of n-ADHD, there was no relationship between MQ and FDI, POI, and PSI, and there were statistically meaningful correlations between the rest of the factors. In the group of ADHD, there was no relationship between FDI and POI, POI and MQ, PSI, POI and PSI. and there were statistically meaningful correlations between the rest of the factors. Moreover, ADHD showed lower levels of EIQ, among which the quantitative score for ‘stroop test' variable was the lowest, while ‘withdrawal effectiveness’ and ‘design fluency' followed next. Regarding the qualitative score, ‘accordance rate of finding missing spots' and index for ‘restraining repetition' of the test for design fluency showed low scores, along with index for ‘interference inhibition' of the test for stroop. These results imply that ADHD have low levels of FDI capability related to deficit of attention, low levels of EIQ, and less capacity to response inhibition, or to working memory what they know compared to n-ADHD. The above results will be of important information in establishing education and learning strategies that consider the recognition characteristics of ADHD, and in inputting arbitration programs.
