신경인지기능 손상 유무에 따른 아동기 ADHD의 증상 특성

초록

본 연구에서는 아동기 ADHD를 신경인지 기능 손상 유무 집단으로 구분하고 그 수행 특성을 비교하였다. 구체적으로, 지능과 공존질환을 통제한 아동기 ADHD를 대상으로 전산화 신경인지 검사와 기타 증상 척도들을 실시하였다. 각 소검사별 수행이 -2 표준편차 이하인 참가자의 경우 손상으로 기록하였으며 세 검사군(주의력, 기억력, 고위인지기능)에서 두 개 이상의 손상을 보인 경우 손상집단으로 분류하였다. 그 결과, 전체 226명이 손상집단으로, 46명이 비손상 집단으로 구분되었다. 결과적으로, 손상집단의 경우 세 검사군 모두에서 현저한 수행 손상이 나타났으나 비손상 집단에서는 다수의 종속측정치에서 손상이 전혀 나타나지 않는 등 상반된 결과가 나타났다. 한국아동인성검사를 비롯한 ADHD 증상 척도들에서는 집단 간 차이가 나타나지 않았다. 많은 연구에서 동일한 신경인지 특성을 가진 집단으로 간주하는 아동기 ADHD 내에서도 검사 수행에서 차이가 나타나며, 표집에 따라 실행기능 연구들에서 혼재된 결과를 일으킬 수 있다는 점이 확인되었다. 향후 관련 연구에서는 아동기 ADHD 표집 및 연구결과 해석에서 신경인지 손상 유무에 따른 특성을 고려해야 할 것으로 생각 된다.

Abstract

In this study, we classified childhood ADHD according to whether there was neurocognitive impairment or not, and compared the characteristics of the performance. Specifically, computerized neurocognitive testing and other symptom scales were administered to childhood ADHD who controlled intelligence level and comobidities. If the performance of each dependent variation is less than -2 standard deviation, it is recorded as the impairment, and participants with two or more impairments in the each categories(attention, memory, and cognitive function) were classified as a impaired group. As a result, a total of 226 persons were classified as impaired group and 46 persons as a non-impaired group. Resultingly, the impaired group showed significant performance impairment in all tests, but there were conflicting results in the non-impaired group such that none impairment was observed in many dependent variation. There was no significant difference between the groups in the KPI-C and the ADHD symptom scales. It has been identified that differences in the performance of neurocognitive test within the childhood ADHD which is considered to have the same neurocognitive characteristic, and sampling can lead to mixed outcomes in executive function studies. Conclusionally, the results of this study should be considered in the sampling and interpretation of childhood ADHD in the future.
