노인장기요양제도의 판정도구 및 케어매니지먼트 :일본 및 미국의 경험을 통하여

본 논문은 우리의 노인장기요양제도의 등급판정도구 및 케어매니지먼트제도에 관하여 문헌고찰에 의거한 탐색적 연구이다. 일본과 미국의 장기요양실시 사례를 우리나라와 비교분석을 통하여 장기요양인정조사표의 개선 방향, 케어매니지먼트의 역사, 역할, 기능 등에 관한 논의를 하고자 한다. 공적 사회프로그램의 역사가 짧은 우리나라에서 충분한 준비기간이 없는 상태에서 제5대 사회보험에 해당하는 노인장기요양제도를 실시하였다. 장기요양보험제도의 시작과 함께 전문인력의 부재, 평가판정의 객관성, 시설의 다양성에 관하여 개선의 목소리가 나오고 있다. 요양보호사제도의 신설 등 관련 직종과 시설의 재정비가 일어나고 있지만, 명확한 평가체제를 위한 객관적 도구의 개발에 관한 지속적 논의가 필요하다. 미국의 MDS는 여러 방면으로 유용하게 사용되며, 단순한 평가판정과 권고 사항에 불과한 장기이용계획서 작성에 그치고 있는 우리의 경우와 대조가 된다. 요양시설을 둘러싼 서비스 질의 개선, 객관적 질 지표 등으로 사용될 수 있도록 개선이 있어야 할 것이다. 본 논문은 다음과 같은 제언을 하고자 한다. 첫째, 등급판정도구인 장기요양인정조사표의 항목에 대해 좀 더 심도 있는 연구가 요구된다. 객관적으로 수치화할 수 있는 항목은 불과 몇 개 되지 않으므로 정확성이 갖추어져야 한다. 각 항목에 대해 우리 실정에 맞는지에 대한 경험적인 검토와 조사도구의 신뢰성 및 타당성이 지속적으로 향상되어야 한다. 둘째, 케어매니지먼트를 담당 할 노인전문인력을 양성하여, 심리사회적 욕구까지도 고려하여 명확한 케어플랜의 수립이 필요하다.
Abstract
The current study examines both instrument and care management by reviewing foreign long-term care systems particularily focusing on Japan and the USA. Despite a short history of public social insurance system, South Korea launched a fifth long-term care insurance from 2008 which is primarily modeled on the Japan's long-term care insurance. Since its inception, several problems are emerged such as lack of professional care workers, inappropriatedness of instrument, and poor facilities. Although much progess has been made about the creation of home helper and increase in long-term care facilities, the development of appropriate instrument designed for eligibility still needs to be improved. America's MDS(Minimum Data Set) has been served as multi-facet purposes. MDS has not only used for determining eligibility, but also improving qualities of care and developing indicators of quality of each long-term care facilities. Two suggestions have been made. First, More research about instrument tool needs to be done to make an accurate decision about eligibility. Second, Geriatric care manager might be trained to plan cares and advocate the elderly clients' unmet needs.
