미국 장기요양 제도의 변천과 케어 매니지먼트의 등장에 관한 연구

초록

본 연구의 목적은 최근 한국의 공적노인요양보장 제도 도입상의 쟁점들을 보다 심도 있게 고찰하기 위해 미국의 장기요양(long-term care)제도의 역사적 변천과정과 케어 매니지먼트 체계의 부상이라는 최근의 경향을 고찰해 보는 데 있다. 미국 제도의 역사적 고찰은 미국과 영국 등에서 1980년 대 이후 출판된 논문과 저서 45편을 통한 문헌 연구로 이루어졌다. 본 연구의 결과를 통해 다음과 같은 미국 장기요양 제도의 경향들을 파악할 수 있었다. 첫째, 오늘날 미국 장기요양 제도의 가장 큰 변화는 장기요양제도의 주요 공적 재원인 Medicaid의 시설 편향적 지원으로 크게 증가된 시설 수요를 지역사회 및 재가 중심요양으로 대체해 나가는 데 있다. 이에 따라 크게 급성 케어와 장기 요양의 통합(integration of acute and long-term care), 재정과 전달체계의 통합(integration of financing and service delivery) 및 재가와 시설요양의 연속적 케어망(continuum of care) 구축이라는 세 가지 차원의 통합이라는 목표 아래 요양정책들이 추진되어 나가고 있음을 파악할 수 있었다. 둘째, 미국 장기요양제도는 의료정책(health care policy)과 밀접한 관계를 가지며 변화해 왔는데, 재정운영과 서비스 전달체계의 통합이라는 managed care정책 틀에서 전통적인 사례관리(case management)가 통합적인 케어 매니지먼트(integrated care management)로 변화되고 있음을 파악할 수 있었다. 미국 장기요양제도 상의 케어 매니지먼트는 전통적인 사례관리 모형의 기능에 지속적인 서비스 패지와 유연한 재정운영의 결합적인 체계로 이루어졌다. 결론적으로 지난 50년간의 시행착오를 걸친 장기요양정책의 전개에 있어 다각적인 변모를 통해 재정의 안정과 케어 매니지먼트의 활용으로 단편화된 서비스 전달체계의 통합을 추진하는 미국 정책들의 최근 변화에 대한 본 연구의 분석은 본격적인 한국의 장기요양제도 실행에 앞서 한국 장기요양제도 관련 설계에 중요 사항으로 고려되어야 하겠다.

Abstract

The aim of this study is to examine the history of long-term polices in US and to analyze the backgrounds of emergence of care management in order to further investigte several controversial issues around a 2004 proposal of Korean national long-term care system. The recent trends of US long-term care policies have been found to be re-organized and re-shaped under the policies objects of 1) integration of acute and long-term care, 2) integration of financing and service delivery, and 3) establishing continuum of care, based on the findings of this study. The managed care, one of main recent changes in US health care system have also significant impact on the emergence of care management that has been regarded as a mixture of traditional case management models and an integrated financing mechanism with a coherent package of community-based long-term care services. The findings of this study suggests that the Korean national long-term care system should be designed through the further analysis on historical lessons from US long-term care polices, particularly financing methods and care management.
