숫자를 (재)생산하는 몸, 도구화된 여성 건강: 저출산 정책으로서의 <난임(불임)부부지원사업>에 대한 고찰

초록

이 글은 대한민국 정부가 저출산 대응의 하나로 2006년부터 시작한 <난임(불임)부부지원사업>을 여성과 아이들의 건강의 측면에서 그리고 실제 효과의 측면에서 분석하였다. 이 연구가 해명하고자 한 연구 질문과 분석 결과는 다음과 같다. 첫째, 이 <사업>은 여성의 건강에 대한 종합적인 고려에서 출발하였는가? 이 <사업>은 협소한 의미에서의 생식 건강에 대해서도 아무런 고려가 전제되지 않았고 그러다보니 여성의 신체적･정신적 건강 그리고 개인적･사회적 건강을 증진하는 데 기여할 수 없다. 오히려 여성의 생식 건강 개념이 축소･변형되는 결과를 가져와, 여성의 몸은 합계출산율의 분모와 분자를 생산 혹은 재생산하는 숫자로 취급되었고 또한 여성의 건강은 ‘저출산 대응’을 위해 도구화되었다. 둘째, 이 <사업>은 저출산의 효과적인 대책이 될 수 있는가? 그리고 이 <사업>은 다른 저출산 정책과 내적 모순을 일으키지는 않는가? 이 사업이 시작되기 전에 불임과 저출산에 대해 그 원인 관계나 결과로서의 효과 측면에서 타당성이 입증된 바 없고 주무 부서 또한 이에 대한 면밀한 고려를 선행한 바 없다. 또한 저출산의 다른 정책과 내적 모순을 일으키고 있는데, 이에 대해 어떤 조사나 연구도 수행된 바 없기에 정책 수행의 통합성을 기대할 수 없어서 과연 ‘저출산 대처’라는 궁극적 목적을 달성할 수 있을 것인지 알 수 없다. 셋째, 이 <사업>은 그 명칭이 가리키는 바 과연 불임부부들에게 실질적 도움이 되는가? 물론 아무런 공적 지원이 없는 상황과 비교한다면 재정 지원은 분명 도움이 되었지만, 지원과 함께 약제비 상승 등으로 인해 비용이 전체적으로 상승하였다. 또한 불임부부의 건강이나 행복추구보다는 숫자의 측면에서 접근함으로써 여전히 당사자들을 대상화시키는 한계를 갖는다.

Abstract

This study aims to analyze the “National Supporting Program for Infertility Couples” (hereinafter the “Program”), started in 2006 as one of the measures taken by the South Korean government to deal with low fertility in Korea, in terms of the health of women and children and in terms of the actual effects. Specifically, this study is to clarify the following research questions:First, did the “Program” start with a comprehensive consideration for women's health? Or rather, has the “Program” caused the concept of women's health (including reproductive health) to change or deteriorate?; Second, can the “Program” be an effective measure to deal with low fertility? And does this “Program” not cause internal contradictions with other low fertility policies?; Third, does the “Program” live up to its name and actually really help couples with infertility? Are the actual beneficiaries the infertile couples or in fact the drug companies and medical institutions?This study attempts to answer these questions by analyzing the documents published by the government regarding the “Program” and low fertility. In addition, it will analyze how national policy deals with women's health in conjunction with the amendments of the “Maternal-Child Health Act” implemented since 2009, and previous analysis on the “Maternal-Child Health Program”.
