연명의료결정제도의 경험과 개선점: 제도 관련자들의 견해를 중심으로

연명의료결정제도가 시행된 이래로 한국 사회에서는 제도의 정착을 목표로 한 다양한 연구가 수행되어왔다. 그간 수행되었던 관련 연구와 사업을 토대로 제도가 광범위하게 적용되고 실제 돌봄을 변화시키는 것을 고려해야 하는 시점으로 사용자들의 경험을 확인하고 적절한 전략을 마련하는 작업이 요구된다. 이 과정에서 시행 3년간 제도 운영에 참여해 온 관련자의 경험은 중요한 기초자료로써 활용 가치가 있다. 본 논문은 향후 연명의료결정제도의 개선안 마련의 기초자료를 마련하기 위하여, 제도 관련자들을 대상으로 한 초점집단면접을 시행하고 그들의 경험을 수집하고 이들이 제안한 개선점을 확인하는 내용으로 구성하였다. 초점집단면접은 제도 관련 기관종사자 그룹, 의료제공자 그룹, 의료사용자 그룹을 대상으로 진행했으며 죽음을 포함한 전반적인 사회환경의 변화와 임종기 문화 간의 관계, 예측 가능한 제도의 변화 및 대처 전략과 관련한 내용에 집중하였다. 자료 분석은 주제분석방법(Braun & Clarke, 2006)을 사용하였다. 연구 결과, 그룹별로 제도종사자의 경우 6인, 의료인의 경우 4인, 의료사용자의 경우 5인으로 구성되었으며, 분석 결과 9개의 하위 주제와 3개의 대주제를 도출하였고, 확인된 대주제는 ‘제도에 대한 정확한 정보 제공의 부족’, ‘제도 시행에 소극적인 의료현장’, ‘제도의 결함에 의해 발생하는 격차’이다. 본 연구는 현장의 목소리를 통해 연명의료결정제도의 개선 방향을 확인하는 중요한 실천적 함의를 지닌다. 연명의료결정제도의 발전을 도모하기 위해서는 제도, 서비스, 사회문화적 측면에서 단·장기적으로 구체적인 개선 목표와 전략을 세워나가야 할 것이다.
Abstract
With the aim of establishing a life-sustaining treatment decision system over the past five years, various studies have been conducted to identify the possible outcome(s) of the system. An appropriate strategy for the future is required as the current system has been widely implemented and changes in actual care need to be considered. To this end, the experiences of medical providers and medical users who have been at the center for using the system are valuable data sources. The purpose of this study is to suggest strategies to prepare for the future of the hereby discussed life-sustaining treatment decision system, which characterizes the current situation based on experiences related to end-of-life care through a focus group interview and draws conclusions from it for the future. The interviews were conducted for three respondent groups (institutional workers, medical providers, and medical consumers), focusing on changes in the overall social environment including death and dying culture, predictable system changes, and coping strategies. Results of the interviews were analyzed using the thematic analysis method (Braun & Clarke, 2006). Study respondents consisted of six institutional workers, four medical providers, and five medical consumers. Nine sub-categories and three main categories were identified based on their interviews. Main categories were “Lack of providing accurate information on the life-sustaining treatment decision system,” “A clinical environment that is passive in using the system,” and “Gaps caused by system deficiencies.” This study has important practical implications for suggesting the direction to develop a life-sustaining treatment decision system through the voice of the field. To improve the system, it is necessary to establish specific goals and plans in terms of systems, services, and socio-cultural aspects.
