연구 목적: 이 연구는 2018년부터 시행되고 있는 연명의료결정제도에 따라 사전연명의료의향서 등록기관에서 상담을 제공하는 상담자를 대상으로 의향서 상담 시 어려움을 겪고 있는 요인을 확인하고 분석하는 것이다. 연구방법: 연구방법은 보건복지부가 지정한 사전연명의료의향서 등록기관에서 활동 중인 5년 이상 경력이 있는 4명의 상담자를 대상으로 반구조화된 설문지를 통한 초점집단면접을 실시하고 녹취록을 작성하여 합의적 질적 연구(consensual qualitative research)를 수행하였다. 연구내용: 이 논문의 연구내용은 사전연명의료의향서 상담자들의 경험을 토대로 사전연명의료의향서 작성 상담과정에서의 핵심주제와 어려움의 요인을 알아본다. 결론 및 제언: 사전연명의료의향서 상담자들은 상담과정에서 정보부족에 따른 어려움, 상담과정에서 발생하는 정서적 소진, 연명의료결정제도에 대한 내담자의 상반된 시각을 경험하고 있었다. 본 연구결과를 바탕으로 사전연명의료의향서 상담을 위한 자료개발과 정서적 소진프로그램, 내담자 유형별 상담교육의 필요성을 제언한다.
Abstract
This study is to identify and analyze the factors that are experiencing difficulties in consultation with counselors who provide counseling at registry agencies for advance statement, in accordance with the life-sustaining medical care decision system that has been in effect since 2018. Four counselors with more than five years of experience at the registry agencies for advance statement designated by the Ministry of Health and Welfare were interviewed in a semi-structured questionnaire and transcripts were prepared to conduct Consensual Qualitative Research. Based on the experiences of counselors for pre-life care intentions, we examine the key topics and difficulties in the counseling process for preparing advance statement. Counselors to advance statement were experiencing difficulties due to lack of information during counseling, emotional burnout during counseling, and conflicting views on the life-sustaining treatment decision system. Based on the results of this study, it suggests the need for decision aid, emotional burnout programs, and counseling education for each type of writter for consultation on advance statement.
