수명이 증가하면서 많은 노인들이 의존상태에서 있으나, 의존도가 높은 환자들에게 어떤 의료를 제공해야 할지는 어려운 문제로 남아있다. 의존상태가 높은노인들은 낮은 삶의 질을 가지고 있으며, 그들의 죽음은 주위 사람들에게 슬픔을주기는 하나 자연스러운 현상으로 받아들여진다. 이런 상황에서 어떤 의료를 환자에게 제공하는 것이 좋은지 결정하는 것은 어렵다. 의사결정의 지침이 될 수 있는 환자의 자율성은 제한되어 있고, 이들에게 어떤 치료가 이득이 되는지 논란이되기 때문이다. 이 논문은 의존도가 높은 노인들(dependent elderly)의 의료에서 숙고할 가치로‘존엄’을 제안하였다. 존엄은 의료뿐 아니라 삶의 영역에서도 당연히 존중되어야 할 가치이다. 하지만 실제 임상 현장에서 의존도가 높은 노인환자와 같은취약한 환자의 의료에서 존엄이 잘 실천되고 있는지 의문시 된다. 치료의 중단이나 지속의 논의에 앞서 환자들이 느끼는 존엄감, 내재적 존엄의 고려와 존중이 이루어지도록 해야 한다. 존엄감은 의미 있는 삶의 구현, 자기 존중감 및 자율성 존중과 관련되어 있으며, 의존상태의 노인에게 이를 실현하기 위해서는 돌보는 사람의 관심과 존중이 필요하다. 내재적 존엄의 측면에서 환자와 돌보는 사람들 사이의 좋은 관계는 중요하다. 노인환자의 의학적 의사결정에 참여하는 사람들은돌봄에서 경험하는 가족들의 부담을 이해하고 이를 줄여줄 수 있는 방법을 찾을수 있도록 해야 한다.
Abstract
Aged population in dependent state has increased in our society withincreasing life expectance of people. But to provide what kind of medicalcare to the dependent elderly remains to be as the difficult problems. Thedependent elderly has low quality of life and their death is accepted asnatural event even if those around one is in sorrow. In circumstances it isdifficult to make a good decision for the dependent elderly. Because theirautonomy has been limited and there is controversy whether a treatmentbecome benefit or not for them. This article suggest the dignity as that value that is considered inmedicine. Dignity is natural value to respect in life as well as in medicine. But in real practice it is doubted if the dignity of vulnerable patient likedependent elderly is respected. Before discussion about withholding andwithdrawing of treatment, we have to be achieved consideration and respectfor sense of dignity and intrinsic dignity in care. Sense of dignity independent elderly is close connection with meaningful life, self esteem andrespect for autonomy. For that, care giver's attention and respect is needed. Aspect of intrinsic dignity, good relationship between patient and care-giveris important. we need to consider caregivers burden in care and search theway for being lessened their burden.
