ICF 장애개념에 근거한 한국 장애출현율 산출기준에 관한 실증 연구 - MDS도구 활용을 중심으로

초록

본 연구는 한국의 ‘손상중심의 장애출현율’ 산출기준의 문제점을 재고하기 위해 ICF장애개념을 반영한 MDS 장애출현율 산출도구를 활용하여 장애출현율을 조사하였다. 특히 MDS-Brief 문항의 중에서도 4000번 모듈(12개 문항)을 활용하여 서울시에 거주하고 있는 약 710명을 대상으로 전화조사를 실시하였다. 그 결과 우리나라 장애출현율은 18.7%로 조사되었다. 장애가 없다고 응답한 사람의 비율은 49.3%, 약간은 12.2%, 중간은 19.3%인 것으로 분석되었다. 동 결과는 2017년 장애인실태조사의 장애출현율 5.39%에 비해 약 3배 이상 높은 비율을 보이고 있는 것으로 나타났다. 이러한 결과는 장애인실태조사의 ‘장애출현율’과 한국의 ‘등록장애인’이 장애인복지정책대상자를 모두 포괄하고 있는지에 대한 논의의 필요성을 제기한다. 즉 장애인실태조사의 장애출현율은 「장애인복지법」의 손상기반 장애인이다. 따라서 등록장애인 수와 대동소이한 값으로 장애출현율이 도출될 수 밖에 없다. 이는 장애환경 변화 및 장애개념을 반영하지 못할 뿐만 아니라, 장애인복지정책의 당국자들도 잠재적 수요자들의 복지욕구에 대해 대응하기 어렵다고 할 수 있다. 따라서 우리나라 손상중심의 장애판정제도에 대한 개편과 잠재적 장애인복지수요자에 대한 정책들이 점진적으로 구축되어야 할 것을 제안한다.

Abstract

This study investigated the disability prevalence using MDS, a tool for calculating based on the concept of ICF disability. In particular, a telephone survey was conducted on about 710 people living in Seoul using module No. 4000 of MDS-Brief questions (12 questions). As a result, 18.7% Korea's disability prevalence was surveyed. The proportion of those who answered ‘no disability’ was 49.3%, 12.2% for ‘Mild’ and 19.3% for ‘Moderate’. The results were about three times higher than the 5.39% tallied in 2017 for the disability survey. It raises the need to discuss the purpose of the survey on the ‘disability prevalence’ of the survey and whether the ‘registered people with disabilities.’ covers all persons subject to the welfare policy for the disabled. The disability prevalence of the survey on the actual conditions of the handicapped is the disability-based 손상 under the Act of Welfare for people with disability. Therefore, disability prevalence can only be derived from the same value as the number of registered people with disabilities. Not only does this not reflect changes in the disabled environment and the concept of disability, but it is also difficult for officials in the welfare policy to respond to the welfare needs of potential consumers. Therefore, it is proposed that policies on the disability assessment system centered on damage in our country should be gradually established.
