라틴아메리카의 의료개혁을 통해 본 신자유주의적 국가개혁의 패턴- 멕시코, 아르헨티나, 칠레의 사례를 중심으로

1980년대 중반부터 시작되어 라틴아메리카의 많은 나라들이 경험한 경제위기로 인해 라틴아메리카 국가들은 신자유주의적 국가개혁을 요구받게 되었다. 신자유주의적 국가개혁은 거대한 국가를 축소하여 국가가 갖고 있는경제, 정치 및 사회적 기능을 축소하는 것을 골자로 추진되었고, 이를 요구한 국제 금융 기구들은 비교적 일관된 정책 제안을 제시하였다. 본 논문은신자유주의적 국가개혁을 위한 정책 제안들이 라틴아메리카 국가들에서 어떻게 적용되었는지를 멕시코, 아르헨티나 그리고 칠레의 의료개혁을 사례로 고찰하였다. 그 결과 본 논문은 비교적 일관되게 요구되었던 신자유주의적 국가개혁이 라틴아메리카 각국의 정치 및 사회적 조건과 만나 서로 다른형태로 전개되었다는 것을 밝혔다. 이는 신자유주의 하의 라틴아메리카를연구함에 있어서 그 다양성에 주목해야한다는 것을 보여주며, 이러한 다양성의 원인을 찾는 연구의 필요성 또한 제시한다.
Abstract
In 1990s, many Latin American countries implemented healthreform designed to increase efficiency of the sector as well as towithdraw the state from it. Such efforts derived from various policyrecommendations made by international financial institutions such asWorld Bank, IMF and Inter-American Development Bank. Theserecommendations contained several specific policy suggestionsincluding privatization, decentralization and deregulation of healthsector. Several Latin American countries accepted the instructionsand the three cases discussed in this paper-Mexico, Argentina andChile- seemed to diligently followed them. However, uniform policyrecommendations have not been implemented in a uniform way. Rather, Latin American countries modified and negotiated theneoliberal state reform and ended up producing distinct policy outcomes, which, this paper argues, are the reflection of political andsocial conditions of each country. Therefore, this paper points outthat neoliberal state reform in Latin America is not a unilateralpolicy pushed by powerful international financial institutions. Asneoliberal health reform policies were contested by political andsocial conditions of each Latin American country and their outcomevaried in each case, this paper emphasizes that neoliberal statereform should be understood in the context of Latin Americanpolitical and social reality.
