청소년 심리평가에서 한국판 MMPI-A의 임상적 유용성에 대한 연구

본 연구는 한국판 MMPI-A가 청소년의 심리적 증상과 행동적 문제들을 평가하는 데에 유용한 지를 알아보기 위해 수행되었다. 정신과에 내원한 202명(남 144명, 여 58명)의 청소년들과 MMPI-A 표준화 연구에 참여했던 한국 청소년 규준집단의 MMPI-A결과를 척도수준과 문항수준에서 비교하였다. 한국 청소년 규준집단과 임상집단의 평균은 약 1/5 표준편차 분리되어 있으며, 전반적으로 한국 청소년 임상집단의 척도 평균이 약간 상승되어 있는 것으로 나타났다. 임상집단과 규준집단 간의 평균차이가 1표준편차에 근접하거나 또는 그 이상을 나타낸 척도는 A-fam이었고, 남녀를 구분했을 때에는 남학생의 경우 A-fam, 여학생의 경우 Pd, Mf, A-aln, A-fam척도에서 effect size가 비교적 크게 나타났다. 각 진단군별로 분류하여 비교했을 때, Ma 척도를 제외하고 해당 진단군의 증상과 관련이 높은 척도들이 높게 상승되었다. 문항수준의 비교에서는 ADHD를 제외한 모든 진단군에서 해당 진단군의 증상을 가장 잘 대표하는 것으로 여겨지는 척도들의 구성문항들이 10개의 최고 변별문항에 포함되었다. 척도수준의 비교결과와 마찬가지로 ADHD 집단의 과잉활동성을 잘 나타내주는 것으로 고려되었던 Ma척도 문항들은 ADHD집단과 규준집단을 가장 잘 변별해주는 10개 문항들에 포함되지 않았다. 본 연구결과는 부분적으로는 MMPI-A가 임상장면에서 각 진단군별로 나타나는 심리적 증상들을 비교적 잘 반영하며 따라서 임상장면에서의 심리진단 및 평가에 유용할 수 있음을 시사한다. 마지막으로 본 연구의 제한점과 후속연구를 위한 제언이 논의되었다.
Abstract
The purpose of this study was to examine the utility of the Korean MMPI-A test used in the clinical assessment of adolescents. The total clinical sample consisted of 202 psychiatric adolescent patients (144 boys and 58 girls). The comparison group was made up of a normative Korean adolescent sample consisting of 775 boys and 759 girls, who participated in the standardization project of the Korean version of the MMPI-A. When raw scale scores were converted to T scores using the Korean norm, the scale means of the Korean clinical sample were slightly higher, with an average of about one fifth SD separation between Korean normative and clinical samples. When broken down by diagnostic group, the highest elevations were mostly found on intended scales, with the exception of Ma. Item level analyses showed that the items discriminating best between diagnostic groups and the normative sample tended to come from the scales considered to represent the symptoms of each diagnostic group. In sum, this study provided promising results regarding the effectiveness of the Korean MMPI-A in clinical settings. However, further validation studies are needed to improve existing scales or to construct scales that tap into and reflect behaviors or symptoms unique to Korean culture.
