안내염 환자에서 유리체내 0.625% 포비돈 요오드 주사 치료 결과

목적: 외인안내염 및 내인안내염에서 유리체내 0.625% 포비돈 요오드 주사의 치료결과를 알아보고자 하였다. 대상과 방법: 2021년 3월부터 2023년 12월까지 안내염으로 유리체내 0.625% 포비돈 요오드 주사를 받은 환자 21명 23안의 의무기록을 후향적으로 분석하였다. 결과: 19명 19안은 외인안내염이었으며 이 중 백내장 수술 후 발생한 안내염이 14안(58.3%)으로 가장 흔하였으며, 유리체내 포비돈요오드 주사치료 횟수는 평균 1.6 ± 0.7회였으며, 전방축농은 평균 2.5 ± 2.1일 후에 사라졌다. 이 중 16안(84.2%)에서 유리체절제술이 필요하였으며 수술 횟수는 평균 1.1 ± 0.5회였다. 최대교정시력(logMAR)은 치료 전 2.06 ± 0.52에서 치료 후 1.00 ± 0.96로 호전되었다(p<0.001). 내인안내염은 2명 4안이었으며 유리체내 포비돈 요오드 주사는 평균 2.5 ± 1.0회(2-4회)였으며 이후 안내염은 호전되었다. 결론: 안내염 환자에게 유리체내 0.625% 포비돈 요오드 주사는 항생제의 대체 치료법으로 잠재적으로 유용하고 효과적일 수 있다고생각된다.
Abstract
Purpose: We evaluated the treatment outcomes of intravitreal 0.625% povidone-iodine (PI) injection in patients with exogenous and endogenous endophthalmitis. Methods: We retrospectively analyzed the medical records of 21 patients (23 eyes) who received intravitreal 0.625% PI injections for endophthalmitis between March 2021 and December 2023. Results: Of the 21 patients, 19 (19 eyes) had exogenous endophthalmitis, with the most common etiology being post-cataract surgery endophthalmitis in 14 eyes (58.3%). The mean number of intravitreal PI injections administered was 1.6 ± 0.7. The mean time to hypopyon resolution was 2.5 ± 2.1 days. Pars plana vitrectomy (PPV) was required in 16 eyes (84.2%), with an average of 1.1 ± 0.5 surgical interventions. The best-corrected visual acuity (BCVA, logMAR) improved significantly from 2.06 ± 0.52 before treatment to 1.00 ± 0.96 after treatment (p < 0.001). Four eyes of two patients with endogenous endophthalmitis received a mean of 2.5 ± 1.0 intravitreal PI injections (range: 2-4). Clinical improvement was observed in all cases. Conclusions: Intravitreal 0.625% PI injection appears to be a potentially effective alternative to conventional antibiotic therapy for the treatment of endophthalmitis.
