유리체절제술 후 발생한 안내염의 시력 결과에 대한 내경계막제거술의 영향

목적: 유리체절제술 후 발생한 안내염 환자를 대상으로 임상적 특징 및 시력 예후에 영향을 미치는 인자에 대해 알아보았다. 대상과 방법: 2004년부터 2013년까지 유리체절제술 후 안내염이 발생한 9명의 의무기록을 후향적으로 조사하여 첫 유리체절제술 시행 전 시력, 안내염으로 치료 전의 시력, 최종 시력, 당뇨 유무, 유리체절제술 시 사용한 기구의 직경, 백내장 수술 동시 진행 여부,내경계막제거술 시행여부, 안내염으로 치료를 시작한 시기, 전방축농 여부, 균 동정 유무 및 종류가 최종 시력에 미치는 영향을 조사하였다. 결과: 9안 모두에서 유리체강내 항생제 주사를 시행하였고, 7안에서는 추가적인 유리체절제술을 함께 시행하였다. 균은 6안에서 동정되었고 5안이 표피포도상 구균, 1안은 대장균이었다. 첫 유리체절제술 시행 전 시력, 당뇨 유무, 유리체절제술 시 사용한 기구의 직경,백내장 수술 동시 진행 여부, 안내염으로 치료를 시작한 시기, 전방축농 여부, 균 동정 유무 및 종류는 최종 시력에 통계적으로 유의하게 영향을 미치지 않았다. 안내염으로 치료 전의 시력이 최종 시력에 유의하게 영향을 미쳤다. 첫 유리체절제술 시 내경계막을 함께제거한 경우 최종 시력이 낮았으며 통계학적으로 유의하였다(p=0.02). 결론: 유리체절제술 후 발생한 감염성 안내염인 경우 시력 예후가 좋지 않으며 초기 시력이 좋을수록 최종 시력이 유의하게 좋다. 첫 유리체절제술 시 내경계막을 제거한 경우 최종 시력이 유의하게 낮았다.
Abstract
Purpose: To evaluate the clinical course and prognostic factors of visual acuity in post-vitrectomy endophthalmitis. Methods: A total of 9 patients diagnosed with post-vitrectomy endophthalmitis between 2004 and 2013 were enrolled in this retrospectivestudy. To evaluate factors affecting final visual acuity, the following was investigated: best corrected visual acuity(BCVA) before first vitrectomy, BCVA at presentation, presence of diabetes mellitus, type of port used on first pars plana vitrectomy,whether cataract operation was performed simultaneously, whether removal of internal limiting membrane was performedat first pars plana vitrectomy, interval period from initial vitrectomy to treatment of endophthalmitis, presence of hypopyon,whether culture was proven, and type of organism isolated. Results: All 9 eyes received intravitreal antibiotic injections and additional pars plana vitrectomy was performed in 7 eyes. Sixeyes were culture-proven. Staphylococcus epidermidis was cultured from 5 eyes and Escherichia coli was cultured from 1 eye. BCVA before first vitrectomy, presence of diabetes mellitus, type of port used on first pars plana vitrectomy, whether cataract operationwas performed simultaneously, interval from initial vitrectomy to treatment of endophthalmitis, whether culture proven,and type of organism isolated did not affect final visual acuity. BCVA at presentation was an important factor for final visualacuity. Removal of internal limiting membrane at initial vitrectomy was a poor prognostic factor in final visual acuity (p = 0.02). Conclusions: Endophthalmitis after pars plana vitrectomy showed very poor final visual acuity and better BCVA at presentationresulted in better final visual acuity. Poor visual acuity was observed in patients who had the internal limiting membrane removedat first vitrectomy.
