초임부에서 유도된 진통 분만의 임상적 고찰

초록

=목적: 초임부에서 유도된 진통으로 분만을 시작한 군의 임상적 특징을 자연 진통으로 분만을 시작한 군과 비교하여 알아보고자 하였다.연구방법: 2004년 8월부터 2006년 7월까지 분만이 이루어진 73명의 초 임부를 대상으로 하였다. 이들 대상 군을 유도된 진통으로 분만을 시작한 군과 자연 진통으로 분만을 시작한 군으로 나누어 비교하였으며, Bishop score를 4점을 기준으로 ≤4점과 >4점으로 나누어 각 경우에서 분만 방법을 비교하였다. 통계 방법으로 Student t-test, Chi-square, 그리고 Fisher’s exact test를 사용하였다.결과: 유도된 진통으로 분만을 시작한 군은 자연 진통으로 분만을 시작한 군에 비해 ≤4점의 Bishop score를 갖는 임신부가 의미 있게 많았고, 입원부터 분만까지의 시간도 의미 있게 길게 나타났다 (p<0.05). 분만 방법은 유도된 진통으로 분만을 시작한 군에서 자연 진통으로 분만을 시작한 군보다 제왕절개 분만이 더 높게 이루어진 것으로 나타났고, 양수 내 태변 착색도 많은 것으로 나타났다 (p<0.05). Bishop score가 >4점인 경우 제왕절개 분만보다 정상 질식 분만이 많았으나 통계적으로 유의한 차이를 보이지 않았다 (p=0.100).
