임신말기 유도분만에서 Misoprostol 경구투여와경질투여 효과에 대한 비교

=목적: 본 연구는 임신말기 유도분만에서 미소프로스톨 100 g 경구 투여시와 미소프로스톨 50 g 경질 투여시의 유도분만 효과 및 안정성을 비교 분석하고자 하였다.연구 방법: 2003년 9월부터 2005년 4월까지 본원 산부인과에서 유도분만을 위해 입원한 임산부 중 80명을 무작위 추출하여 100 g의 미소프로스톨을 경구 투여 또는 50 g의 미소프로스톨을 경질 투여하였다. 이러한 두 군간에 질식분만까지의 소요시간, 유도분만중 자궁경관의 숙화정도, 제왕절개율의 빈도 그리고 태아 및 산모에 미치는 영향 등을 비교하였다. 결과: 양군간에 연령, 분만력, 임신주수, 키, 체중 등은 유의한 차이가 없었고, 유도분만 4시간 후, 8시간 후에 시행한 내진검사에서 Bishop 점수는 경구 투여군이 경질 투여군보다 더 유의하게 높았다 (5.9±1.4 vs. 5.1±1.1, p<0.05; 8.6±2.3 vs. 7.1±2.1, p<0.05). 질식분만까지의 소요시간은 초산부의 경우 경구 미소프로스톨 투여군이 경질 미소프로스톨 투여군보다 약 200분 가량 빨랐다 (927.4±344.7분 vs. 1130.6±297.0분, p<0.01). 경산부의 경우에서도 경구 미소프로스톨 투여군이 경질 미소프로스톨 투여군보다 약 150분 가량 더 빨리 분만이 이루어졌다 (674.2±110.4분 vs. 831.1±181.6분, p<0.01). 분만방법에 있어서 양군간에 유의한 차이는 없었으며, 제왕절개술의 빈도에 있어서도 경구 미소프로스톨 투여군에서 8예 (20%), 경질 미소프로스톨 투여군에서 11예 (27.5%)로 양군간에 유의한 차이는 보이지 않았다. 분만과정에서 발생할 수 있는 부작용인 빈맥 수축, 과도긴장, 과자극증후군, 태아 심박동이상, 양수내 태변배설등에서 양군간에 유의한 차이는 보이지 않았다. 출생시 태아의 체중 및 1분, 5분 Apgar 점수에서 차이는 통계적으로 의미가 없었다.
