자기공명영상으로 확진한 임신 20주경의 진행성 복강임신 환자에서 수술 후 잔류태반의 초음파 추적검사

산전진찰을 한 번도 받지 않던 임신 20주경의 산모에서 초음파검사와 자기공명영상으로 수술 전에 복강임신을 진단하고 성공적으로 치료하였다. 진행성 복강임신을 수술할 때에는 태반의 제거 시에 대량 출혈의 위험성이 높으므로 태반을 억지로 분리하지 말고 남겨 두는것이 좋다. 또한, 진행성 복강임신의 진단을 놓치지 않기 위해서는 초음파검사를 할 때 태아 및 태반을 둘러싸고 있는 자궁벽의 존재를 확인하는 것이 중요하다.
Abstract
We successfully treated a case of advanced abdominal pregnancy which was diagnosed with ultrasonography and magnetic resonance imaging in a woman of 20th week of pregnancy who did not receive any prenatal care. It is advisable to leave the placenta in situ because removing the placenta may be associated with severe hemorrhage. Furthermore, identifi cation of uterine wall around the fetus and the placenta is very important in diagnosing abdominal pregnancy.
