공여자 유래 세포 유리 DNA (dd-cfDNA)는 이식 후 거부반응을 조기에 검출할 수 있는 새로운 바이오마커로 주목받고 있다. 국내 임상현장에서 dd-cfDNA 검사의 임상적 유용성은 아직 충분히 규 명되지 않았다. dd-cfDNA 검사의 활용이 활발해질 것으로 예상됨 에 따라 이식 장기 거부반응을 예측하기 위해서는 변화하는 dd- cfDNA 패턴과 기준값의 이해가 필수적이다. 본 종설에서는 dd- cfDNA 검출 방법, 다양한 이식 장기에서의 dd-cfDNA 변화 양상, 그리고 dd-cfDNA 시행 및 해석 전략을 종합적으로 다루었다.
Abstract
Donor-derived cell-free DNA (dd-cfDNA) has emerged as a novel biomarker for the early detection of post-transplant rejection. However, its clinical utility remains insufficiently characterized in Korea. As the adoption of dd-cfDNA testing is expected to expand, establishing an understanding of dy- namic dd-cfDNA patterns and reference values will be essential for accurately predicting transplant organ rejection. This review discusses the meth- ods for dd-cfDNA detection, its dd-cfDNA variations across transplant organs, and the strategies for test implementations and result interpretation.
