Abstract
Background: High plasma levels of donor-derived cell-free DNA (dd-cfDNA) indicate allograft injury and serve as a molecular genetic marker for identifying graft rejection in kidney transplant recipients. In this study, we evaluated the potential of plasma dd-cfDNA in monitoring and predicting graft dysfunction in patients undergoing kidney transplant. Methods: We measured plasma dd-cfDNA concentrations using AlloSeq cfDNA in patients who underwent kidney transplantation more than 10 years ago at the Jeonbuk National University Hospital. The significant threshold for plasma dd-cfDNA concentration was set at 0.5%, based on a multicenter study of 1,092 kidney transplant recipients that validated dd-cfDNA levels of ≥ 0.5% as indicative of clinically apparent and subclinical rejection. The dd-cfDNA concentrations were measured in 21 patients who underwent kidney transplantation more than 10 years ago with avail- able clinical information and in vitro diagnostic test results. Results: Seven of the 21 patients were clinically suspected of having chronic renal allograft dysfunction (CRAD). Applying the 0.5% threshold for plasma dd-cfDNA concentration to assess CRAD, 14 patients showed levels below this cutoff, among them two were suspected of having CRAD. Seven patients had levels ≥ 0.5%, five of whom were suspected of having CRAD. Comparing the two groups, the relative risk of CRAD in patients with plasma dd-cfDNA concentrations ≥ 0.5% was 5, which was statistically significant (95% confidence interval, 1.28-19.60; P = 0.0209). Conclusions: This study demonstrated a significant association between plasma dd-cfDNA levels and CRAD in kidney transplant recipients, sug- gesting its potential for evaluating the renal function status in these patients. Further large-scale cohort studies are required to validate the role of plasma dd-cfDNA in monitoring allograft function.
