Abstract
Purpose: This study was conducted to evaluate the efficacy of proximal femoral nail (PFN) for intertrochanteric fractures. Materials and Methods: From October 2000 to December 2004, 82 cases of intertrochanteric fracture were treated with proximal femoral nail. Mean duration of follow-up was 31 months. The mean age was 72.8 years and there were 28 males and 54 females. Such factors as the operating time, the amount of blood loss(Hemovac count) and transfusion, duration of hospital stay, mean duration to bone union, neck-shaft angle, lag screw sliding by follow-up radiograph, ambulation time, intra-operative and post-operative complication and functional status were analyzed. Results: The average operation time was 54 minutes, the mean amount of transfusion was 320 ml, and postoperative mean Hemo-vac count was 77 ml. The mean hospital stay was 19 days, and mean duration of radiologic bone union was 13.7weeks. The average neck-shaft angle on immediate postoperative X-ray was 132.1 degrees and 129.7 degrees at the last follow-up(average varus displacement, 2.4 degrees). The average amount of fracture impaction was 4.2 mm. The average partial weight bearing started 3.3 days after operation, and full weight bearing was permitted at 9 weeks after surgery. Complications were hip pin and neck screw protrusion for 2 hips, superficial infection for one, loss of neck-shaft angle more than 5 degrees for 4, intra-articular cutting out of neck screw for one, and Z-effect by excessive displacement of femur neck screw and hip pin for one hip. Conclusion: The proximal femoral nail could be preferable for the fixation of intertrochanteric fracures after the long term follow-up because of reducing operating time, blood loss, low complication rates and favorable postoperative function.
