Abstract

This study aimed to examine the effects of a 12-week dementia prevention exercise program(twice a week, 60 minutes per session) on physical fitness and cognitive function in older adults with physical disabilities who are capable of walking. To achieve this objective, a preliminary application of the dementia prevention exercise program—including facial muscle exercises, hand play, flexibility, aerobic, resistance, coordination(balance), and dual-task exercises—was conducted with two older adults with physical disabilities. Based on the results, the program was modified and refined by integrating overlapping activities. The final program was then implemented with 20 older participants with physical disabilities capable of walking, divided into an experimental group(n=10) and a control group(n=10). Over 12 weeks, the experimental group participated in the program twice a week for 60 minutes per session. To evaluate the exercise effects, physical fitness(muscular function: grip strength, chair sit-to-stand; balance: 3m target turn, functional reach test; cardiopulmonary endurance: 6-minute walk test; coordination: figure-eight walking, finger-to-nose test) and cognitive function(Korean version of the Mini-Mental State Examination, MMSE-K) were measured. Collected data were analyzed using two-way repeated measures ANOVA to examine the interaction effects between groups and time points. The results showed significant interaction effects(p<.05) between group and time in physical fitness factors, including muscular function, balance, cardiopulmonary endurance, and coordination. Additionally, significant interaction effects were found in the total MMSE-K score(p=.038) and the attention and calculation domain (p=.038). These findings suggest that regular exercise can help older adults maintain physical fitness necessary for independent daily living and improve cognitive function, thereby preventing dementia.
