의료소비자권리와 공유의사결정에 대한 연구

본 연구는 의료소비자권리의 필요도와 보장수준은 어떠한지 알아보고, 보장수준에 따라 의료소비자권리는 어떻게 나뉘는지를 살펴보았다. 또한 최근 이용한 의료서비스에서의 공유의사결정 수준과 그 영향요인을 알아보고, 이러한 공유의사결정 수준의 매개효과를 중심으로 의료소비자권리 보장수준에 영향을 미치는 요인을 알아봄으로써, 의료소 비자 주권 향상에 기여하고, 의료소비자교육의 기초자료를 제공하고자 했다. 본 연구는 20~50대 남녀 소비자 619명을 대상으로 인터넷 조사를 실시해, 다음의 결과를 얻었다. 첫째, 의료소 비자권리 중에서 비밀보장권의 보장수준이 가장 높았고, 알 권리, 건강권, 선택권 순으로 잘 보장되고 있다고 응답 한 반면, 불만제기 및 보상받을 권리와 참여권의 보장수준은 낮았다. 둘째, 가장 최근에 이용한 의료서비스에서의 공유의사결정 수준은 3.47점(5점 만점)으로, 소비자와 의료진 사이의 의사소통이 비교적 잘 이루어지고 있음을 알 수 있었다. 셋째, 공유의사결정 수준에 대한 위계적 다중회귀분석을 실시한 결과, 의료소비자적 특성과 의료공급자 측면의 특성만이 공유의사결정 수준에 영향을 미쳤고, 의료소비자적 특성 중에서는 주관적 소비자역량의 영향력이, 의료공급자 특성 중에서는 소비자친화적 의사 태도 변수의 영향력이 가장 컸다. 넷째, 의료소비자권리 보장수준에 대한 경로분석 결과, 가장 큰 영향력을 가진 변수는 소비자친화적 의사 태도였고, 권리의 필요도, 공유의사결정 수 준의 순으로 그 영향력이 컸다. 이상의 연구결과를 바탕으로 다음의 결론을 도출했다. 첫째, 소비자들은 본 연구에서 추가로 제시한 참여권, 안전 할 권리, 불만제기 및 보상받을 권리 등도 필요하다고 인식하고 있었으므로, 『보건의료기본법』 상의 의료소비자권리 에참여권, 안전할권리, 불만제기및보상받을권리등을추가할필요가있다. 둘째, 의사가치료대안과함께각 대안의 장단점(혜택, 위험, 비용 등)도 함께 설명해야 의료진과 소비자 간 원활한 공유의사결정이 이루어질 수 있으 므로, 의사에게는 치료 대안과 함께 각 대안의 장단점도 함께 설명하도록 주지시키고, 소비자에게는 의사에게 치료결 정과 관련된 정보를 적극적으로 요구하도록 하는 의료소비자권리 교육이 필요하다. 셋째, 공유의사결정 수준에 영향 을 미치는 변수 중에서 소비자친화적 의사 태도 변수의 영향력이 가장 컸는데, 이러한 결과는 공유의사결정 수준 향 상을 위해 소비자와 적극적으로 소통하고자 하는 의료진의 태도 변화가 필요함을 의미한다. 넷째, 환자로서의 권리 (건강권, 안전할 권리, 비밀보장권)의 보장수준을 향상시키기 위해서는 정부의 적극적인 개입이 필수적이고, 소비자 로서의 권리(알 권리, 선택권, 참여권, 불만제기 및 보상받을 권리)의 보장수준을 높이기 위해, 소비자는 평상시 꾸 준히 건강에 대한 관심을 가지고, 소비자로서의 주관적 역량을 증진하기 위한 노력을 기울여야 하며, 의료서비스 이 용시치료결정과정에적극참여해야한다. 또한의료공급자는소비자가의료서비스이용시에편안함을느낄수있 도록 소비자친화적 태도를 함양하고, 소비자의 알 권리 충족을 위해 설명의무를 다하며, 소비자와의 적극적인 의사소 통을 통해 그들의 결정 회피 유인이 제거되도록 돕고, 소비자가 선택권을 충분히 행사할 수 있도록 독려해야 한다.
Abstract
The purpose of this study is to find out what the level of healthcare consumer rights need and guarantee, and how healthcare consumer rights are divided according to the level of guarantee. In addition, by examining the level of Shared Decision-making in medical services and its influential factors, and by examining the factors that influence the level of healthcare consumer rights assurance, In order to provide basic data of healthcare consumer education. We conducted an internet survey on 619 male and female consumers in their 20s and 50s, and the results of statistical analysis are summarized follows. First, as a result of examining the extent of assurance of healthcare consumer rights, it was found that the level of assurance of confidentiality rights was the highest, and that it was guaranteed in the order of right to be informed, the right to health care without discrimination and to choose. The guarantee level of rights to complain and redress, the right to participate was low. Second, the level of shared decision - making in the most recently used medical service was 3.47 out of 5 points, and it was found that the communication between the consumer and the medical staff is relatively well done. Third, hierarchical multiple regression analysis on the level of Shared Decision-making showed that only the characteristics of healthcare consumer and medical provider except socio-demographic characteristics influenced the level of Shared Decision-making. Among the healthcare consumer characteristics, the influence of subjective consumer competency was the greatest, and there was a big difference when compared with the influence of the consumer friendly attitude of physician. Fourth, as a result of the path analysis on the level of assurance of healthcare consumer rights, the most influential variables were consumer friendly attitude of physician, necessity of rights, and level of Shared Decision-making.
