RhD 양성 농축적혈구 35단위 수혈 후 Anti-D가 검출되지 않은 RhD 음성 1예

초록

현재 수혈 전에는 ABO형, RhD형, 항체선별검사, 항체확인검사와 교차시험을 시행하는데, 과거에 비해 이 검사들에 대한 오류율은 감소하였지만 여전히 발생하고 있다. RhD형의 경우 RhD 음성자가 RhD 양성 적혈구에 노출되면 anti-D가생성되며 anti-D는용혈성 수혈부작용과 신생아용혈성질환의 원인이 될 수 있 다 . 그러므로 RhD 음성자에게 RhD 양성 혈액을 수혈하는 것은 제한되어야 한다. 저자들은 8년 전 RhD 양성으로 오인되어 35단위의RhD 양성 농축 적혈구를 수혈받은 RhD 음성 환자에게서 anti-D가 검출되지 않은 1예를 경험하였다. 이 증례는 Anti-D 시약을이용한 RhD 검사와 표준 약-D 검사에서 RhD 음성형으로 분류되었으며RHD 유전자에 대한 multipex PCR with sequence-specific priming에서 RhD 음성임을 확인하였다.
