분노조절 문제를 동반한 ADHD 아동의 놀이치료 사례연구

본 연구는 분노조절의 어려움과 공격성을 동반한 초등학교 1학년 남아에 대한 놀이치료 사례이다. 아동은 ADHD로 진단된 후 2년 째 약물치료 중으로, 잦은 분노 폭발과 또래 및 주변 사람들에 대한 빈번한 공격적 행동으로 인해 또래로부터 거부되고 가까운 성인들로부터 지속적인 부정적 피드백에 노출되어 왔으며, 학교에서의 적응에도 심각한 수준의 어려움을 겪고 있었다. Full Battery 심리검사를 실시한 후, 2014년 7월부터 2015년 8월까지 주 1회 회기별 40분씩 놀이치료를 42회 진행했으며, 아동에 대한 치료 후에는 부모상담을 실시하였다. 아동은 놀이치료 과정에서 유기 불안, 소외감과 고립감, 통제에 대한 욕구를 드러냈으며, 강한 공격성을 표출한 후, 손상된 자아감을 회복하려는 노력을 보였다. 놀이치료를 통해 내적인 부정적 정서를 충분히 발산하면서 카타르시스를 경험한 아동은 자아를 강화하는 단계까지 나아갈 수 있게 되었으며, 정서를 수용가능한 방식으로 적절히 표현할 수 있게 되었다. 이와 동반하여 가정이나 학교에서의 적응력도 향상된 것으로 나타났다. 또래관계 향상을 위한 사회성 집단치료를 계획한 후 개별 놀이치료는 종결되었다.
Abstract
The purpose of this study was to investigate the therapy progress and play theme of a 8-year old boy with ADHD and anger problem in child-centered play therapy. The child has showed inattention, hyperactivity and impulsivity, and suffered from relational difficulty because of yelling, hitting and anger explosion. The child has taken medication since diagnosed with ADHD. The researcher conducted 42 session play therapy once per week. The results were as follows: First, the child's aggressive and disruptive behavior decreased, and self-regulation improved. Second, examining of the changes of play theme at stage, the play theme of power, control, and abandonment were mainly expressed in initial stage. And then, the theme of aggression, power and revenge were performed. Finally, ego-strenthening and mastery were expressed. The effectiveness of child-centered play therapy was confirmed for a child with ADHD and anger problem. The pattern of play theme was discussed.
