Abstract
-Purpose:This study aims to analyze the structure of international health cooperation for global vaccine development and procurement and to derive effective cooperation strategies based on the principles of equity in vaccine access, supply chain stability, and sustainability of governance. In particular, it focuses on the need to establish an integrated cooperation framework across the entire vaccine lifecycle, as vaccines have been re-recognized as strategic assets in terms of national and economic security in the post-COVID-19 era. -Methods:An explanatory sequential mixed-methods design was applied. A systematic literature review was conducted in accordance with PRISMA guidelines, analyzing 50 relevant publications. Based on the identified cooperation domains, a structured survey using a 5-point Likert scale was administered to 15 experts. The findings from the literature review and survey were then integrated to derive strategic frameworks. -Results:Strengthening domestic vaccine production capacity (4.27) was identified as the highest policy priority, followed by strengthening cooperation with international organizations (4.13) and establishing an integrated support system across the vaccine lifecycle (4.13). Expansion of international collaborative research and clinical trials (4.07) and global vaccine procurement cooperation (4.00) were also considered important. Based on the integrated analysis, six strategic areas were identified: (1) multilateral and bilateral leadership in cooperation, (2) development of a vaccine R&D ecosystem, (3) establishment of a procurement network, (4) strengthening of the global value chain, (5) development of a governance model, and (6) phased implementation strategies. -Conclusion:Global vaccine cooperation should be implemented based on a multi-layered governance framework integrating policy coordination, financing, research and development, and production functions. In particular, an integrated approach combining production-based cooperation with supply chain diversification is required to overcome the limitations of procurement-centered strategies. This study is limited by the small expert sample size and cross-sectional design, and future research should validate the proposed strategies through Delphi studies and longitudinal analyses.
