Abstract

-Background and Objectives:As Korea enters a super-aged society, health screening non-participation among older adults constitutes a critical public health challenge. Screening rates decline sharply with age—from 69.2% among those aged ≥65 years to 42.7% among those aged ≥85 years—yet the multidimensional determinants of this disparity remain insufficiently understood. This study aimed to examine the effects of social support and digital literacy on health screening non-participation among older adults, and to determine whether these effects differ across subgroups defined by frailty status, cognitive function, and area of residence. -Methods:Secondary data from the 2023 National Survey of Older Koreans (N=9,955, aged ≥65 years) were analyzed. Binary logistic regression was performed to assess the independent effects of social support and digital literacy on non-participation, adjusting for sociodemographic characteristics, older adult characteristics, and regional environmental factors. Interaction terms were incorporated to test moderating effects, followed by supplementary stratified analyses by moderator levels. -Results:Overall, 22.1% of participants reported non-participation in health screening within the preceding two years. Significant risk factors in the multivariable model low digital literacy (OR=2.23), cognitive impairment (dementia) (OR=2.10), no chronic diseases (OR=1.54), low social support (OR=1.40), and frailty (OR=1.30). Stratified analyses revealed that the effect of low digital literacy was most pronounced among frail older adults (OR=2.60), demonstrating that the convergence of digital and health vulnerability substantially amplifies non-participation risk. The composite vulnerability analysis showed that individuals with overlapping vulnerabilities exhibited the highest risk of non-participation in health screening, and the risk was further amplified by low digital literacy. -Conclusion:These findings underscore the need for targeted interventions, including expansion of home-visit and accompanied health screening services for socially isolated older adults, maintenance of offline reservation channels for digitally vulnerable populations, and strengthening of mobile screening services in rural areas.
