Abstract

-Purpose:This study examined sociodemographic, behavioral, and regional predictors of walking and moderate-to-vigorous physical activity (MVPA) adherence among Korean adults, based on health and functional status. -Methods:Data (n=231,714) from the 2024 Korea Community Health Survey were analyzed. Survey-weighted multivariate logistic regression models were employed to estimate average marginal effects (AMEs). Models included sociodemographic, behavioral, and standardized regional contextual factors, alongside interaction terms with health and functional status. -Results:Among 231,714 adults in the 2024 Korea Community Health Survey, 25.9% had hypertension or diabetes and 17.6% had ICF-based functional limitations. Physical activity was consistently lowest in the disability group. Regional variation was substantial, particularly for walking (≈25%p range). Individuals with chronic diseases showed higher walking but lower MVPA than the non-reported group, whereas the disability group had markedly lower probabilities for both walking (–17.2%p) and MVPA (–13.2%p). Walking increased with age, while MVPA declined. Contextual factors were influenced differently: population density favored walking, whereas park area and regional income favored MVPA. Individuals with disabilities appeared more sensitive to regional environmental conditions. -Conclusion:There were clear disparities on physical activity levels based on individuals’ health and functional status, with the ICF disability group showing the lowest compliance. The influence of area-level factors varied significantly by activity types and subgroups. Therefore, for physical activity promotion policies to be effective, strategies must differentiate the expected outcomes of walking and MVPA, requiring tailored resource allocation and design based on subgroup needs and local context.
