우리나라 성인의 미 충족의료 현황: 연령 간의 차이를 중심으로

초록

1989년 이후로 전 국민에 대한 의료보장제도가 시행되었음에도 불구하고, 높은 비율의 환자본인부담은 필요한 의료서비스의 적절하게 이용하는 데 장애가 될 수 있다. 본 연구는 필요한 의료서비스에 대한 미 충족 현황과 이와 관련된 요인을 파악하고 연령에 따른 차이를 살펴보고자 하였다. 2005년 국민건강영양조사 자료를 사용하여, 19세 이상 성인 25,215명을 3개의 연령 그룹(19-39세, 40-64세, 65세 이상)으로 나누어 분석하였다. 의료에 대한 미 충족 경험은 응답자 스스로 판단한 것으로, 우리나라 성인의 17.1%가 경험한 것으로 나타났다. 연령과 상관없이 공통적으로 나타난 미 충족의료의 주요 요인은 소득과 건강상태였고, 젊은 연령에서는 건강상태에 따른 차이가 크게 나타난 반면, 노인층에서는 소득수준에 따른 영향이 크게 나타났다. 미 충족의료를 개선하기 위한 정책방안으로, 우선 전반적인 건강보험 보장성을 개선하는 것이 필요하고, 노인층과 청장년층의 미 충족의료의 특성을 구체적으로 파악하여 접근하여야 할 것이다.

Abstract

This study investigated the prevalence of unmet needs for health care among Korean adults and related factors. Using data from the 2005 National Health and Nutrition Examination Survey, 25,215 adults who aged 19 and over and three age groups (19-39, 40-64, and >=65) were analyzed to identify. unmet needs identified by the respondent. The logistic regression analysis was employed to examine factors associated with unmet needs and the estimate from the logistic regression was transformed into the probability and predictive margin, i.e., relative risk.Overall, 17.1% of respondents experienced unmet needs for health care among adults. Unmet needs mainly depend on income and health status, number of chronic conditions in particular, across age groups. Income was most strongly associated with unmet needs among the elderly while the number of chronic conditions was most important among young adults.More efforts should be made to meet needs for health care by relieving out-of-pocket cost burden, and further research that investigates unmet needs depending by age groups would suggest helpful policy implications.
