한국 노인 미충족 의료 관련 요인의 체계적 고찰

초록

노인 미충족 의료는 의료 접근성 및 형평성을 나타내는 지표로 최근 중요한 사회적 문제로 대두되고있다. 본 연구는 한국 노인의 미충족 의료 관련 요인을 분석한 선행 연구 결과를 종합하고, 향후 연구 방향에 대해 제언하는 것을 목적으로 한다. 이를 위해 연도 제한 없이 국내외 학술지에 출간된 관련 문헌들을 모두 검색하였고, 본 연구에서 설정한 기준에 부합한 문헌 18편을 분석하였다. 14편 연구가 횡단면으로 설계되었으며 일부 연구만 종단 연구 설계가 이루어졌다. 미충족 의료 요인의 경우 여성, 저학력, 저소득, 나쁜 주관적 상태가 일관되게 유의한 요인이었으며, 경제적 이유로 인한 미충족 의료나 기타 원인으로 인한 미충족 의료는 각각 다른 요인과 상관관계를 보였다. 한국의료패널이 가장 많이 사용된 자료원이었으며 미충족 의료 경험 비율과 경제적 이유가 원인인 미충족 의료율은 2012-2014년 지속해서 줄어드는 것을 확인할 수 있었다. 향후 다양한 원인으로 인한 미충족 의료를 대상으로 하는 연구가 필요하며, 방법론 측면에서는 더욱 엄밀한 분석 방법을 통해 인과관계를 분석한 연구가 활발히 이루어져야 할 것이다.

Abstract

Unmet health care needs act as an indicator for medical accessibility and health equity; recently, number of related research has been increasing. The objective of the study is to critically review literatures of unmet health care needs among older people aged 65 and above in South Korea, and make recommendation for future research. We performed a systematic review of papers for entire year and identified 18 studies eligible for inclusion. Most studies analyzed the determinant factors cross-sectionally and only a few were analyzed longitudinally. Main determinants of unmet health care needs were female, low-education, low-income, poor subjective health status. Having unmet healthcare needs due to economic reason or other reasons showed associative factors that are different from each reason. “Korea Health Panel Study” was most frequently used data source and rate of experiencing unmet health needs and considering economic hardship as barrier of unmet health needs decreased consistently from 2012 to 2014. We suggest future studies to investigate reasons of unmet health care other than economic hardship to analyze the determinant factors. In methodological aspect, more causality studies should be done with precise method.
