의료급여 수급가구의 의료이용 변화에 관한 연구: 2007년 의료급여 1종 외래 본인일부부담제도 도입을 중심으로

과도한 진료비 상승을 억제하고 의료급여 수급가구의 의료이용을 적정 수준으로 통제하기 위하여 2007년 의료급여 1종 외래진료에 한해서 본인일부부담제도가 도입되었다. 본 연구는 전술한 본인일부부담제도의 도입에 따른 의료급여 1종 수급가구의 의료이용 변화를 건강보험 가입가구와 비교하여 건강보험 가입가구 대비 의료급여 수급가구의 상대적인 과다 의료이용 여부를 고찰하는 데 목적이 있다. 본 연구는 의료급여 수급가구가 건강보험 가입가구보다 의료적 필요도가 높기 때문에 발생하는 의료서비스 이용에 대한 부분을 통제하기 위해 성향점수매칭(Propensity Score Matching, PSM) 기법을 이용하여 2005-2009년 기간 동안의 표본에 대하여 두 집단의 인구, 사회 경제적 특징 및 건강상태를 통계적으로 동일하게 맞춰주는 매칭 작업을 실시한 후, 매칭이 이루어진 표본에 한정하여 회귀분석을 실시하였다. 분석 결과 본인일부부담제도 시행 전에는 건강보험 가입가구 대비 의료급여 수급가구의 외래진료 이용횟수가 약 11회 더 많았으나, 제도 시행 후에는 두 집단 간 통계적으로 유의한 차이가 발견되지 않았다. 이는 본인일부부담제도의 도입이 의료급여 수급가구의 의료이용을 적정 수준으로 통제하려는 정책 목표를 일부 달성했다고 하는 주장을 지지하는 결과로 평가할 수 있다.
Abstract
In order to suppress the excessive medical care use of the medical aid beneficiaries, Korea Medical Aid Program for the poor underwent a fundamental reform in 2007. One of the most significant aspects of the reform was the introduction of co-payment system for out-patient services. This paper aims to analyze the change of the medical care use of the medical aid beneficiaries after the reform. Specifically, we assess the effects of the introduction of co-payment system for out-patient services on the medical care use of the beneficiaries, by estimating the amount (i.e. number of the out-patient and in-patient services) of medical care use of the aid beneficiaries, as compared to that of Korea National Medical Insurance subscribers. Since medical aid beneficiaries and medical insurance subscribers differ not only in socio-economic status but health conditions, we employ a Propensity Score Matching (PSM) for the sample of the two groups during period 2005 - 2009, and then conduct a regression analysis. We find that while the aid beneficiaries had out-patient treatment about 11 times more than the insurance subscribers before the reform, there were no statistically significant difference between the two groups after the reform.
