고령층 외래 진료 본인부담 제도 변화가 의료 이용에 미치는 영향

2018년부터 고령층의 외래 진료에 적용되던 본인부담이 진료비 15,000원~20,000원 구간에서 30%에서 10%로 경감되었다. 이 연구는 건강보험 보장성 강화 정책으로 고령층의 외래 진료 본인부담이 경감되며 고령층의 외래 이용 행태가 어떻게 변화했는지 분석하고자 했다. 한국의료패널 2012~2018년 외래 서비스 이용 자료를 활용하여, 제도 변화 후 진료비 15,000원을 초과하는 진료 건수가 증가했는지 여부와 건강 상태에 따른 외래 진료 이용 변화를 검토했다. 또한, 집군 분석을 통해 진료비 15,000원의 기준점을 중심으로 의료 이용의 행태가 변화했는지 통계적으로 검증했다. 그 결과, 2018년 이전에는 진료비 15,000원 아래로 집군 현상이 관찰되었으나, 2018년 이후 해당 집군 현상은 상당부분 해소되었음을 정량적으로 확인했다. 로지스틱 회귀 분석을 통해 2018년 제도 변화 이후 진료비 15,000원을 초과하는 진료비 발생 확률이 높아짐을 보였다. 특히, 주관적 건강상태가 좋지 않은 고령층 환자들이 진료비 15,000원을 초과하는 진료를 받는 확률이 높아졌음을 확인했다. 이중차분 모형을 통해서 정책 대상이 아닌 비고령층과 대비하여 진료비 15,000원 초과의 외래 이용이 증가했음을 재확인했다.
Abstract
Since 2018, out-of-pocket payments for the elderly outpatient services costing from 15,000 KRW to 20,000 KRW in clinics in Korea were reduced from 30% to 10% of total spending. This study analyzes how this out-of-pocket change affects outpatient service utilization of the elderly. Analyzing Korea Health Panel Survey from 2012 to 2018, I identified whether the elderly are more likely to utilize services over 15,000 KRW (the threshold at which an out-of-pocket payment increased significantly until 2017) and the sicker elderly patients are benefited after the policy change. In addition, a bunching mass just before the threshold is examined to identify whether the mass disappears after the 2018 policy change. Logistic regression results show that the elderly patients, especially the sicker elderly measured by self-assessed health status, are more likely to utilize services over 15,000 KRW. The bunching mass at the notch until 2017 also disappeared, meaning that the financial burden at the threshold 15,000 KRW lessened for the elderly. The difference-in-difference estimation results confirm that the finding is robust, comparing changes in outpatient visits among the elderly and the non-elderly.
