제네릭 의약품의 국가 간 약가 비교: 분석방법별 약가 수준의 차이 고찰

외국과의 약가 비교연구는 사용된 방법에 따라 다양한 결과를 보이는 것으로 알려져 있는데, 본 연구는 2016년 기준 우리나라와 외국 15개국의 제네릭 약가수준을 여러 분석방법을 사용하여 비교하였다. 외국 약가 자료원은 Parma Price Information (PPI)과 일본 노동후생부 관보를 사용하였고, 건강보험청구금액을 기준으로 상위 성분 중, 제네릭이 존재하면서 비교국가가 3개국 이상 존재하는 정제 23개 성분을 대상으로 하여, 대상 국가와 1:1 비교하였다. 연구결과 우리나라 제네릭 의약품의 약가수준은 분석조건에 따라 다소 차이가 있었지만, 전반적으로 외국에 비해 높은 것으로 나타났다. 외국과 달리 우리나라에서 제네릭 약가는 브랜드의약품과 비슷한 수준에서 형성되어 있고, 제네릭 출시 시기, 제네릭 점유율, 경쟁품목 수 등은 제네릭 약가 수준에 영향을 미치지 않는 것으로 나타났다. 본 연구의 분석방법 중 결과에 영향을 크게 미치는 것은 비교국가 선정과 사용량 가중치, 그리고 가격유형이었다.
Abstract
To investigate generic drug price levels in Korea, we conducted a cross-national drug price comparison. Also, it was investigated whether the price level could differ when the price comparison method was changed. Parma Price Information (PPI) and Ministry of Health, Labor and Welfare (MHLW) official gazettes were used for 14 European countries and Japan respectively. Bilateral comparisons with 15 countries were conducted for the top 23 molecules based on health insurance claims data. As a result, the price level of generic drugs in Korea was slightly different depending on the method, but it was overall higher than that of comparison countries. The weighted average price of generics was similar to that of the brand, and none of the launch date, the market share, and the number of competing items appear to affet the drug price level. This study has demonstrated the sensitivity of cross-national drug price comparisons to the methods used, including comparison countries, weights, and price type.
