본 연구는 외국인 유학생들의 의료이용 결정요인을 확인하는 것을 목적으로 한다. 이를 위해 고등교육기관에 재학 중인 국내 외국인 유학생을 대상으로 실시한 대규모 전국 단위의 설문조사를 자료원으로 이용하여 의료비용과 의료이용의 빈도를 분석하였다. 분석 결과, 외국인 유학생들의 체류기간이 길어질수록 의료서비스 이용량이 증가하였으며, 한국어 구사력이 뛰어난 경우에도 의료서비스 이용량이 증가하는 것으로 나타났다. 또한 유학생 중 해외보험 가입자는 국내 민간보험 가입자보다 의료서비스 이용량이 적게 나타났으나, 국민건강보험 가입자는 국내 민간보험 가입자와 유의한 이용량의 차이가 없었다. 이는 외국인 유학생들의 체류기간이 늘어날수록 내국인과의 의료이용 패턴이 닮아가 이용량이 늘어나게 되며, 한국어 구사력이 뛰어날 경우 언어로 인한 의료이용 제약이 감소하기에 의료서비스 이용량이 늘어나는 것으로 해석된다. 또한 국내 의료보험 가입 종류는 의료비 지출에 유의미한 영향을 미치지 않는 것으로 나타났는데, 이는 청년층이 다수인 외국인 유학생의 특성상, 입국 후 질병 치료를 위해 특정 보험에 선택적으로 가입한다거나 가입 후 의료이용량이 크게 변하는 경우가 적은 것이 그 이유일 수 있다. 이러한 결과는 기존에 주로 분석되어온 성별, 연령 등의 요인 외에도 체류기간, 한국어 구사력 등 외국인의 특성을 반영한 요인들이 의료서비스 이용에 영향을 끼치며, 향후 외국인들의 의료이용 패턴과 건강보험 가입자 증가의 영향을 가늠하는 데 있어 외국인 고유의 특성들이 반영되어야 함을 보여준다.
Abstract
The purpose of this study is to identify the determinants of health care utilization of international students enrolled in higher education institutions in South Korea. We analyze the determinants by using two-part model and a large-scale survey data. The use of health care services increased as the length of stay in Korea and the proficiency of Korean language increased. International students who enrolled in international insurance use less health care service compared to those who enrolled in domestic (Korean) private insurance, however, there was no significant difference between the enrollees in domestic private insurance and national health insurance (of Korea). As foreign students live longer in Korea, they tend to follow Korean’s life pattern and this may cause the use of health care services increased. When the proficiency in Korean becomes excellent, the language barrier to health care utilization decreases. In addition, foreign students are relatively younger and healthier, which causes health care utilization less sensitive to insurance type. These results show that the unique characteristics, such as the length of stay and proficiency in Korean, should be considered in analyzing health care patterns of foreigners.
