ADHD 진단을 위한 전산화 신경인지 기능검사의 유용성

초록

ADHD의 진단 및 치료효과 평가를 위한 검사도구로서의 가치를 알아보기 위해 120명의 정상아동 집단, 31명의 ADHD집단 및 16명의 ADHD가 아닌 소아정신장애 집단을 대상으로 하여 세 집단 간의 전산화 신경인지기능검사 결과를 비교하였고, 그 변별력을 알아보았다.CNT의 주의력검사와 기억력검사 그리고 고위인지기능검사에서 ADHD집단의 경우 대체로 정상집단과는 유의하게 차이가 있었으나 비ADHD집단과는 유의한 차이가 없었다. CNT의 정상아동 집단과 ADHD집단 및 비ADHD집단에 대한 판별분석에서 정상아동에 대한 특이도가 99.0%, ADHD에 대한 민감도가 55.6%, 비ADHD에 대한 민감도가 66.7% 그리고 전체 판별 정확도가 89.5%임을 확인하였다. 따라서 CNT는 ADHD의 진단적인 보조도구로서 유용하며, 경과관찰 및 치료효과를 정량적으로 볼 수 있는 유용한 도구로 생각한다.

Abstract

Objective: The focus of this paper is to examine the utility of the Computerized Neurocognitive Function Test(CNT) system for the diagnosis and assessment of therapeutic effect in children with Attention Deficit Hyperactivity Disorder(ADHD).Method: The Computerized Neurocognitive Function Test and Korean Kaufman Assessment Battery for Children were performed to 120 normal children, 31 children with ADHD, and 16 children with other neuropsychiatric disorders. And then compared with attention, memory and higher cognitive function among three groups.Result: Children with ADHD had significant differences compared to normal children in attention, memory and higher cognitive function, but there were no differences between children with ADHD and children with other neuropsychiatric disorders. In discriminant analysis for CNT among three groups, the specificity to normal children was 99.0%, the sensitivity to children with ADHD was 55.6%, the sensitivity to children with other neuropsychiatric disorders was 66.7%, and the accuracy of all groups was 89.5%.Conclusion: This study suggested that CNT is relatively valuable method and in the evaluation of children with ADHD. But further verification studies will be necessary.
