목적: 전방내 위치한 아메드 방수유출장치 삽입수술과 이와 병합한 수술을 시행 후 그 효과 및 임상경과에 대해 살펴보고자 한다. 대상과 방법: 2003년 6월부터 2012년 8월까지 본원에서 아메드 방수유출장치 삽입술과 병합된 수술을 시행 받은 환자 중 1년 이상경과관찰이 가능하였던 67명, 71안을 대상으로 후향적 연구를 시행하였다. 결과: 유리체절제술, 초음파 수정체 유화술 및 인공수정체 삽입술과 아메드 밸브 삽입술이 동시에 시행된 군에는 16명(16안), 아메드밸브 삽입술만 시행한 군에는 51명(55안)이 포함되었다. 두 군에서 술전에 비해 의미있게 안압이 하강하였으나, 두 군간 차이는 없었다(병합/단일 수술군, p=0.008/0.000). 수술 전 사용한 안압하강제의 평균개수는 두 군에서 모두 의미있게 감소하였으며(p=0.002/0.000),시력(logMAR)은 술전에 비해 술후 호전되었으나 통계적 의의는 없었다(p=0.309/0.052). 결론: 유리체 망막질환이나 수정체 혼탁이 동반된 난치성 녹내장 환자가 병합수술을 시행한 경우 안압 저하를 위한 단일 장치 삽입술을 시행한 경우와 비교하여 안압 저하와 시력 호전, 사용 안약 수의 변화에서 동등한 수준의 결과를 보여주었으며, 합병증의 빈도또한 증가하지 않아 일차적인 치료로 고려할 수 있다.
Abstract
Purpose: To evaluate surgical results after combined operation associated with Ahmed glaucoma valve (AGV) implantation. Methods: A retrospective review was performed for 71 eyes of 67 patients who underwent combined operation associated withAGV implantation from June 2003 to August 2012. Results: Pars plana vitrectomy or phacoemulsification combined with AGV implantation were performed for 16 eyes and AGVimplantation alone was performed in 55 eyes. Mean IOP (mm Hg) and the number of antigluacoma agents for both the combinedand single groups showed a statistically significant decrease (combined/single; p = 0.008/0.000, 0.002/ 0.000). Visual acuity (logMAR) was improved, but these differences were not statistically significant (combined/single; p = 0.309/0.052). Conclusions: Combined vitrectomy or phacoemulsification and AGV implantation for intractable glaucoma with vitreoretinal diseaseor cataract is considered to be the primary procedure because of its equivalent efficacy in lowering IOP, improving visualacuity and decreasing the number of glaucoma agents.
