특발성 망막앞막에서 유리체황반견인증후군 유무에 따른 수술 결과

초록

목적: 특발성 망막앞막에서 술 전 유리체황반견인증후군 동반 유무에 따른 술 전 황반 구조의 변형 및 술 후 결과를 알아보고자 하였다. 대상과 방법: 특발성 망막앞막으로 유리체절제술, 망막앞막제거술 및 내경계막제거술을 시행 받은 54안(51명)을 대상으로 의무기록을후향적으로 조사하였다. 술 전 빛간섭단층촬영으로 유리체황반견인증후군 동반 유무에 따라 2군으로 나누어 술 전 황반 형태 변형을알아보고, 술 후 결과 및 예후인자를 분석하였다. 결과: 특발성 망막앞막만 있는 군은 29안, 유리체황반견인증후군이 동반된 군은 22안이었다. 유리체황반견인증후군이 동반된 군에서술 전 층판황반원공의 발생이 많았으며, 술 전 시력도 유의하게 낮았다. 하지만 술 후 시력 호전 정도는 더 커서, 술 후 최종 시력은두 군에서 유의한 차이가 없었다. 술 전 불규칙한 망막표면, 망막비후, 층판황반원공, 낭포성 황반부종 등은 술 후 시력에 영향을 미치지 않았으며, 술 전 시력 및 시세포 내절/외절 접합부 파열 여부가 술 후 시력 회복에 의미가 있었다. 결론: 특발성 망막앞막에서 유리체황반견인증후군 유무에 관계없이 유리체절제술, 망막앞막제거술 및 내경계막제거술 후 유의한 시력호전을 보였으며, 술 전 시세포 내절/외절 접합부의 파열 여부가 망막앞막 수술의 시력회복에 더 의미가 있는 것으로 생각한다.

Abstract

Purpose: To present preoperative macular structure deformities and surgical outcomes in idiopathic epiretinal membrane (ERM)patient according to the presence of vitreomacular traction syndrome (VMTS). Methods: This retrospective observational study included 54 eyes of 51 patients with idiopathic ERM, who underwent pars planavitrectomy with ERM and internal limiting membrane (ILM) peeling, and were followed for more than 6 months. Subjects wereclassified into 2 groups according to the presence of VMTS using preoperative optical coherence tomography (group 1: ERMwithout VMTS, 29 eyes; group 2: ERM with VMTS, 22 eyes). Preoperative macular structure deformity and surgical outcomeswere compared and prognostic factors were analyzed. Results: Group 1 (ERM without VMTS) included 29 eyes, and group 2 (ERM with VMTS) included 22 eyes. Group 2 had a significantlyhigher occurrence of lamellar macular hole and a significantly lower preoperative best-corrected visual acuity (BCVA)compared to group 1. However, final postoperative BCVAs were not significantly different between the groups because BCVAchanged more in group 2. Preoperative macular structure deformities (irregular retinal surface, retinal thickening, lamellar macularhole, and cystoid macular edema) did not affect visual outcomes. The presence of photoreceptor inner segment/outer segment(IS/OS) junction disruption and preoperative BCVA were highly related with visual outcomes. Conclusions: BCVA significantly improved in idiopathic ERM patients who underwent pars plana vitrectomy with ERM and ILMpeeling regardless of the presence of VMTS. There may be a stronger association of IS/OS junction disruption with visual acuitycompared to VMTS in idiopathic ERM surgery.
