목적: 특발성 망막전막에서 유리체 절제술 시행 전 후 망막의 두께와 시력 변화를 알아보고 최종 시력에 영향을 미치는 인자를 알아보고자 하였다. 대상과 방법: 특발성 망막전막으로 유리체 절제술을 시행하고 1년 이상 추적 관찰하였던 62명 62안을 대상으로 수술 전후 시행한 시력과 빛간섭단층촬영 결과를 분석하였고 술 후 시간 경과에 따른 망막중심두께 및 시력의 변화를 알아보았다. 또한 최종 시력에 영향을 미치는 인자를 상관분석을 통해 분석해 보았다. 결과: 최대 시력 호전은 술 후 1개월부터 3개월 사이에서 관찰되었고, 이후 술 후 9개월까지 지속적인 시력 호전이 관찰되었다. 중심망막두께는 12개월 동안 지속적인 감소를 보였고, 가장 많은 감소는 수술 후 1개월째에 관찰되었다. 최종 시력과 수술 전 시력은 유의한 상관 관계(r=0.635, p＜0.001)를 보였고, 다른 인자들은 의미 있는 상관 관계를 보여주지 않았다. 결론: 망막전막 수술 후 시력 호전 및 망막중심두께 감소가 관찰되었으며 이는 각각 술 후 9개월, 1년까지 지속되었다. 술 후 최종 시력은 수술 전 시력과 유의한 관련이 있었으며, 수술 전 시력이 예후 인자로서 중요할 것으로 생각한다.
Abstract
Purpose: To evaluate the clinical outcomes of visual acuity and foveal thickness after vitrectomy for an idiopathic epiretinal membrane (ERM). Methods: We retrospectively reviewed the records of 62 patients (62 eyes) with ERM who had been treated with vitrectomy between 2004 and 2009. Visual acuity and central macular thickness from optical coherence tomography imaging were obtained preoperatively and at every postoperative follow-up visit. Results: Mean preoperative visual acuity and central macular thickness were 0.495 ± 0.292 log MAR and 414.645 ± 95.528 μm, respectively. Mean visual acuity and central macular thickness 1 month after surgery were 0.389 ± 0.373 log MAR and 341.484 ± 73.676 μm, respectively. Visual acuity improved within 9 months and central macular thickness significantly decreased 12 months after surgery. Most of the changes in visual acuity and central macular thickness took place during the first 3 months. The only parameter which was significantly correlated with final visual acuity was preoperative visual acuity (0.635) (p < 0.001). Conclusions: Visual acuity and central macular thickness improved 12 months months after vitrectomy in patients with idiopathic ERM. Preoperative visual acuity had a significant correlation with final visual acuity.
