초록

본 연구는 많은 국가들의 공통적인 관심주제로 부각되고 있는 장기요양서비스 품질관리제도의 모습을 규명하는 목적으로 수행되었다. 14개국 15개 장기요양서비스 품질관리제도 사례를 유형화 하여 그 특징을 분석하고, 유형별 소속 국가들의 품질관리제도에 대한 유사점과 차이점을 밝혀내고 이것이 무엇을 의미하는지를 탐색해 보고자 하였다. 이를 위해 국가비교연구와 유형화 작업에 유용한 퍼지셋 이상형 분석 방법을 활용하였다. 품질관리제도에 대한 이론적․실제적 검토를 통해 이용자 중심성, 관리 실효성을 국가의 품질관리제도 성격을 구성하는 요소로 도출하였다. 14개국 15개 사례에 대한 분석 결과, 3개의 유형(참여적집중관리형, 집중관리형, 낮은관여형)에 사례들이 소속되어 있음을 확인할 수 있었다. 또한 품질관리제도의 모습이 각 국가의 제도적 전통에 기반하여 다양한 모습으로 존재한다는 사실을 발견할 수 있었고, 이와 관련한 영향요인으로 국가주도성, 시장화·민영화 수준, 전통적 시장의 비중, 제공기관의 지위, 시설서비스 비중, 서비스현금지급제도의 비중 등을 확인하였다.

Abstract

This study was carried out with the aim of identifying the appearance of long-term care quality regulatory schemes that is emerging as a common topic of many countries. 15 quality regulatory scheme in the 14 countries are typed and its features are also analyzed to find the similarities and differences of the quality regulatory schemes in types of country belonging and try to explore what they mean. For this purpose the comparative studies and Fuzzy set ideal type analysis have been utilized. Through theoretical and practical review for the quality regulatory schemes, the user-involvement and management effectiveness are drawn for components that make up the character of the quality regulatory schemes. The result of analysis of the 15 cases in 14 countries shows that the cases belong to three types: participatory concentrated- management type, concentrated management type, low-involvement type. The result of analysis shows quality regulatory schemes exist in various forms on the basis of institutional traditions of each country, and influential factors are the levels of national initiative, privatization and marketization, the share of traditional market, the status of provision agencies, the share of residential care and the proportion of cash payments system service.
