사회서비스 현금지급제도의 이용자 선택권 비교연구

초록

본 연구는 퍼지셋 이상형분석방법을 사용한 유형화를 통해 8개국 11개 사회서비스 현금지급제도의 이용자 선택권을 비교한 것이다. 이론적이고 실질적인 지식을 바탕으로 현금급여사용의 융통성과 정보 및 지원시스템의 기능정도를 유형화 지표로 추출하였다. 각국 제도에 대한 분석결과, 현금급여사용의 융통성이 크면서 정보 및 지원시스템의 기능도 강한 이용자지원 예산모형에는 영국의 개인예산제도와 미국의 자기주도지원 프로그램이 속했다. 현금급여사용의 융통성은 크지만 지원시스템이 발달하지 않은 개인책임 예산모형에는 오스트리아 장기요양수당, 독일 현금급여, 이탈리아 동행수당이 속했으며, 현금급여사용의 융통성은 적지만 지원시스템은 발전된 이용자지원 고용모형에는 미국 C&C, 스웨덴 활동지원수당, 네덜란드 개인예산제도가 속했다. 현금급여사용의 융통성도 적고 지원시스템도 발달하지 않은 개별책임 고용모형에는 프랑스 장기요양수당, 영국 직접지불제도, 독일 사례관리예산이 속했다. 유형분석결과 사회서비스 현금지급제도는 기존 사회 돌봄의 틀을 유지하면서도 독자적인 제도로 발전하는 것으로 나타났다. 또한 우리나라 제도 도입 시 이용자 선택권 정도에 대한 함의를 도출할 수 있었다.

Abstract

This study is to compare the user’s choice of 11 cash-for-care in 8 countries by adapting fuzzy-set ideal type analysis. On the basis of theoretical and practical knowledge about cash-for-care, flexibility of cash usage and the functioning of support system are selected as indicators. As a result, UK Personal Budgets and US PM-LTSS pertain to ‘user support budget scheme’that has much cash flexibility and support function. Austria Pflegegeld, Germany Long-term care cash benefits, and Italy IdA pertain to ‘individual responsibility budget scheme’ that has much flexibility of cash usage but does not develop the functioning of support system. US Cash and Counseling, Sweden Personal assistance, and Netherlands PGB belong to ‘user support employment scheme’ that has little flexibility of cash usage but considerably develops the functioning of support system. France APA, UK Direct payments, and Germany case-managed care budgets belong to ‘individual responsibility employment scheme’that has not cash flexibility and support function. Cash-for-care seems to have common features with social care but have its own pictures. It is also necessary to integrate market approaches which can facilitate service user’s choice and anti-market schemes which can act against the negative effects of market approaches.
