본 연구는 문헌을 통해 비판적 사고와 임상적 추론에 대한 개념과 간호에서의 연구와 적용 실태에 대해 살펴보고, 종양간호 임상현장에서 임상적 추론능력이 잘 적용될 수 있는 방안에 대해 고찰하였다. 종양간호는 임상간호 영역 중 대상자나 치료, 간호의 모든영역이 가장 복잡하여 고도의 임상적 추론이 필요한 분야이다. 이런 이유로 종양간호사는 개인의 비판적 사고력과 임상적 추론력을개발하기 위한 노력이 필요하다. 비판적 사고와 임상적 추론 능력은교육과 노력을 통해 증진이 가능하다. 구체적으로, 좋은 임상적 추론을 위해서는 좋은 질문을 할 수 있어야 한다. 그리고 그 질문에 대한 답을 반드시 찾는 것이 중요하다. 이를 위한 개인적 차원의 노력으로 스스로 묻고, 확인하고 검증하는 생각의 습관이 종양간호의수준을 발전시키는 가장 중요한 요소라 생각한다. 그리고 병원 조직 차원에서도 근무 여건과 교육의 기회 제공 등의 지원이 필요하다. 임상추론능력 증진을 위한 다양한 교육과 컨퍼런스나 사례발표를 통한 간호과정 적용 교육이 지속적으로 필요하다. 마지막으로교육 현장에서는 간호사에게 요구되는 임상적 추론능력이 준비된간호사를 교육시키기 위한 비판적 사고력 증진교육과 이를 임상에적용할 수 있도록 심도 있는 실습교육이 이루어 져야 할 것이다.
Abstract
Purpose: The purpose of this study was to determine the concept of critical thinking and clinical reasoning and to propose a strategyto improve clinical reasoning for oncology nurses. Methods: A literature review was used. The concept of clinical reasoning and criticalthinking in nursing was reviewed. Results of research of its area in Korea was evaluated. Results: A summary of the literature reviewfor critical thinking, clinical reasoning, and research of critical thinking in clinical nurse in Korea was described. The concept ofclinical reasoning was not established in nursing and thirteen articles were reported from 2000 to 2014 in Korea. Conclusion: Severalstrategies to improve the clinical reasoning in oncology nursing setting were proposed. First, personal effort to develop thought abilityby using good questions. Second, nursing administration should offer support to the oncology unit to relieve the burden and tochange the nursing delivery system to fit the oncology unit. Finally, nursing school has to teach teach critical thinking in the juniorgrade and develop clinical reasoning ability in the senior grade of clinical practice. it is necessary to teach not only the right practicesbut also the right questions and reasoned thinking to novice nurses during training by preceptors.
